Study On The Diagnostic Value Of Echogenic Gastrointestinal Ultrasound Aid For Gastroduodenal Diseases

Abstract/Summary:
Objectives:TUS-OCCA is a new application of ultrasound examination method which has been used in the clinical diagnosis of gastric and duodenal diseases.The Ultrasound filling aid has being used in this application is Echoic ultrasound aids that can form an echogenic structure in the gastrointestinal tract similar to that of a parenchymal organ whence being oral administer.Echoic ultrasound aids can effectively eliminating the interference of mixed contents such as gas and liquid in the lumen of the stomach and duodenum,allowing the gastroduodenal tract to produce uniformly distributed strong echogenicity,clearly displaying the multi-layered gastric wall structure and its lesions.This application also improve the resolution and detection rate of lesions which can provide a reliable basis for clinical treatment planning.Further more,this application has some advantages that included but not limited to non-invasiveness,non-radiation,high repeatability and low cost,etc.Therefore,this application can be used as a routine examination method for gastric and duodenal diseases in clinical practice.This study further discusses the diagnostic value of echogenic gastrointestinal ultrasound cochlearance(TUS-OCCA)in the diagnosis of common diseases of the stomach and duodenum,and provides reference suggestions for the clinical application。Methods:From 1,January 2020 to 31,October 2022,The Gastroenterology and ultrasound department of One Grade IIIA hospital had been treated one hundred and eighty patients with gastrointestinal tract diseases which including 93(51.7%)males and 87(48.3%)females,with the age distribution from 21-79 years.This article is based on the 180 samples as study subjects.The 180 patients with gastrointestinal diseases enrolled in the study underwent an echogenic gastrointestinal ultrasound co-modulation followed by gastroscopy.The results of the echogenic gastrointestinal ultrasound study and gastroscopy were recorded and analyzed for consistency,comparing the diagnostic compliance,sensitivity and specificity of the two examinations.Results:1.Comparison of the results of Echoic cellulose-based gastric ultrasound contrast agent and Gastroscopy: the number of cases with the same diagnosis in the same group of diseases in both examination modalities were 87 cases of chronic gastritis,12 cases of gastric ulcer,10 cases of duodenal ulcer,19 cases of gastric cancer,4 cases of gastrointestinal polyps,7 cases of special diseases of the gastrointestinal tract and 5 cases of normal images;the diagnostic compliance rates of various diseases were: 94.8% for chronic gastritis,96.45% for The compliance rate of various diseases were: chronic gastritis 94.8%,gastric ulcer 96.45%,duodenal ulcer97.04%,gastric cancer 96.45%,gastric polyp 96.45%,gastrointestinal tract specific tumour 98.22%,and the compliance rate of normal examination result was 91.73%.2.Analysis of the consistency between the results of Echoic cellulose-based gastric ultrasound contrast agent and gastroscopy: the Kappa analysis values of chronic gastritis,gastric cancer and special gastrointestinal diseases were all between0.81 and 1,suggesting that Echoic cellulose-based gastric ultrasound contrast agent and gastroscopy were almost identical in the diagnosis of these diseases;the Kappa analysis values of gastric ulcer and duodenal ulcer were all between 0.60 and0.81,suggesting that echogenic Gastrointestinal ultrasound and gastroscopy are highly consistent in the diagnosis of ulcerative diseases;gastrointestinal polyps have Kappa analysis values between 0.41 and 0.60,suggesting moderate agreement between echogenic gastrointestinal ultrasound and gastroscopy in the diagnosis of gastrointestinal polyps;the diagnostic results of cases with normal echogenicity on ultrasound have Kappa analysis values between 0.21 and 0.40,suggesting the compliance rate of normal diagnostic results between Echoic cellulose-based gastric ultrasound contrast agent and gastroscopy was low.The cases in which ultrasound showed normal echo results compared with gastroscopic results were 4cases of chronic gastritis,2 cases of gastric ulcer,2 cases of early gastric cancer,1patient with special diseases of the gastrointestinal tract and 4 cases of gastric polyps.3.Shortcomings of Echoic cellulose-based gastric ultrasound contrast agent:TUS-OCCA may not allow direct visualization of the lesion due to the separation of the abdominal wall,peritoneal muscles and other organs.Echo-type gastrointestinal ultrasound filling examination has some difficulty in the differential diagnosis of early gastric cancer,microscopic ulcers and lesions less than 7 mm in diameter in higher locations in the gastrointestinal tract,such as gastric polyps,and the diagnosis rate of microscopic lesions in the stomach and duodenum still needs to be improved,and the exact diagnosis of such diseases still requires gastroscopic biopsy.CONCLUSION:1.The diagnostic value of echogenic gastrointestinal ultrasound aids in the diagnosis of common gastric decubitus diseases is obvious as it has a high rate of compliance with gastroscopic findings.2.Echoic cellulose-based gastric ultrasound contrast agent examination has high compliance with gastroscopy in the diagnosis of chronic gastritis,peptic ulcer,gastric cancer and gastric smooth muscle tumour,and its sensitivity and specificity are both high.3.Echoic cellulose-based gastric ultrasound contrast agent can be used as a complementary method to gastroscopy.
