Purpose:To investigate the diffusion and perfusion in normal bone marrow and acute leukemia(AL)bone marrow of lumbar vertebrae in children by using intravoxel incoherent motion(IVIM)and diffusion-weighted imaging(DWI).Materials and Methods:The study group comprised 78 individuals including 50 normal children and 28 AL patients.MR imaging was performed in supine position on GE 3.0T superconducting magnetic resonance.DWI and IVIM-DWI imaging were performed on them,and ADW4.6 workstation was used to process the image.So as to compare the difference of parameters of different sex and age in normal children,the difference between acute lymphoblastic leukemia(ALL)and acute nonlymphoblastic leukemia(ANLL),ALL and normal bone marrow,the correlation between AL parameters and the percentage of bone marrow primordial cells.Results:The ADC,D,D*and f values of normal boys were(0.35±0.09)×10-3mm2/s,(0.15±0.05)×10-3mm2/s,(18.57±12.61)×10-3mm2/s and 47.58±19.23%,and the ADC,D,D*and f values of normal girls were(0.35±0.10)±10-3mm2/s,(0.14±0.04)×10-3mm2/s,(20.93±13.07)×10-3mm2/s and 46.25±19.56%.There was no significant difference in ADC,D,D*and f values among different genders in normal children(P>0.05).No significant differences were found in ADC,D,D*and f values among different age groups in normal children(P>0.05).The D values of the bone marrow showed a negative correlation with age(r=-0.158,P=0.014)in normal children,but there was no significant difference in other parameters(P>0.05).The ADC,D,D*and f values of ALL were 0.32(0.27?0.39)×10-3mm2/s,0.24(0.21?0.28)×10-3mm2/s,8.09(4.29?13.93)×10-3mm2/s,48.98±21.12%,and the ADC,D,D*and f values of ANLL were 0.39(0.29?0.49)×10-3mm2/s,0.21(0.18?0.25)×10-3mm2/s,17.70(5.68?25.20)×10-3mm2/s,45.08±19.04%.There were significant differences in ADC,D,D*values between ALL and ANLL(P=0.006,0.006,0.009),but no significant difference was found in f values(P=0.298).The D and D*values of 1?5 years old ALL group were 0.24(0.22?0.28)×10-3mm2/s,8.31(4.81?14.18)×10-3mm2/s,and the D and D*values of 2?5 years old normal children were 0.14(0.12?0.17)×10-3mm2/s,16.80(11.10?27.30)×10-3mm2/s.There were significant differences in D and D*values between 1?5 years old ALL group and 2?5 years old normal children(P=0.000,0.000).The ADC,D and D*values of 6?10 years old ALL group were(0.32±0.07)×10-3mm2/s,0.24(0.21?0.28)×10-3mm2/s,4.82(3.05?9.75)×10-3mm2/s,and the ADC,D and D*values of 6?10 years old normal children were(0.36±0.09)×10-3mm2/s,0.13(0.11?0.16)×10-3mm2/s,18.45(8.89?29.88)×10-3mm2/s.There were significant differences in ADC,D and D*values between 6?10 years old ALL group and 6?10 years old normal children(P=0.027,0.000,0.000).The ADC and D values showed a negative correlation with the percentage of bone marrow primordial cells(r=-0.253,P=0.003;r=-0.366,P=0.000)in AL group.Conclusion:The values of ADC,D,D*and f values of lumbar bone marrow in normal children were not related to different genders and different age groups.D value could reflect the changing trend and physiological information of normal bone marrow.The values of D and D*could truly reflect the differences of bone marrow diffusion and perfusion between ALL and ANLL,ALL and control group.The values of ADC and D were correlated with the percentage of bone marrow primordial cells in AL,and the D value could better reflect the diffusion of bone marrow under pathological condition.
