BackgroundThe Nineteenth National Congress of the Communist Party of China has put forward that we should resolutely win the battle of poverty alleviati on and continue to adhere to the policy and strategy of precise poverty a lleviation.Solving the problem of poverty caused by illness and returning to poverty caused by illness is a key step to win the battle of poverty alleviation.In recent years,the state has attached great importance to rural issues,continuously strengthened and improved the overall constructi on of the social security system,and comprehensively strived to build a multi-level social security system that benefits the people,integrates urb an and rural areas,has clear rights and responsibilities,guarantees reas onable and moderate and sustainable development.With the rapid change of social and economic situation,the rapid development of urbanization and the dual economic level of urban and rural areas,the imbalance in medical security between urban and rural residents has become increasingly promi nent.Integrating the new rural cooperative medical system with the urban residents'medical insurance system in order to ensure that the rural popu lation in China can better access to basic medical and health services,ac tively solve the outstanding problems of poverty caused by illness and re turning to poverty,and narrow the gap between urban and rural areas.ObjectiveBy using the micro-data of Guangdong Province's Precision Poverty All eviation Archives,this paper evaluates and analyses the alleviating effec t of China' s urban and rural residents' health insurance policies on rural residents' poverty caused by illness and returning to poverty caused by i llness from the perspective of farmers,providing reference for improving the concept of Precision Poverty Alleviation Policy and formulating polic ies to promote rural economic and agricultural development.MethodFirstly,the relationship between health insurance,health and poverty is analyzed theoretically,and then the poverty alleviation effect of heal th insurance for urban and rural residents in Guangdong Province is studi ed empirically.Using a causal inference method based on counterfactual in ference model,propensity score matching(PSM),this paper compares the dif ferent performance of poverty alleviation effect of poor households under the two conditions of participating in urban and rural residents' medical insurance and not participating in urban and rural residents' medical in surance.ResultAmong the poor families caused by illness,the per capita disposable income of insured poor households increased by about 3%compared with the income of uninsured poor households.The total medical reimbursement fees of insured households for basic medical insurance of urban and rural res idents are 7 times that of uninsured poor households caused by illness.At the same time,this study also finds that the general descriptive statist ical analysis overestimates the impact of urban and rural residents' healt h insurance on farmers' health insurance reimbursement fees and per capit a income of households' products because it does not take into account the impact of other confounding factors and the heterogeneous selection bias between poor households.ConclusionMedical insurance for urban and rural residents has no significant ef fect on the improvement of working ability and wage income of poor househ olds;it has a positive but not obvious effect on the per capita disposab le income of poor households;and it has a significant impact on the reim bursement of basic medical insurance for urban and rural households.In o rder to improve the system,we should pay attention to improving the over all level of medical insurance funds for urban and rural residents;stren gthen the linkages between basic medical insurance and disease insurance systems to ensure the labor capacity of the poor;improve the accessibili ty level of primary health services;introduce supporting security for po verty alleviation,give full play to the macro-guiding role of policies;and integrate and enhance the service capacity of medical insurance agencies.
