ObjectiveAccording to the bone mineral density measurement,the differences of bone metabolism indexes P1NP,?-CTX,N-MID,VITD and bone mineral density of knee osteoarthritis with different bone mass in postmenopausal women were compared,and the KL graded bones of postmenopausal knee osteoarthritis patients were analyzed.Metabolic indicators P1NP,?-CTX,N-MID,VITD and bone mineral density,explore the correlation between bone metabolism and the onset and progression of post-menopausal knee osteoarthritis,and analyze different bone mass knee osteoarthritis after menopause The correlation between TCM syndrome distribution and bone metabolism in patients.MethodsA total of 303 postmenopausal women,269 knee osteoarthritis patients,and 34 normal controls were included in the study.All subjects were included in the 1-4 lumbar vertebrae and left total hip average bone by dual-energy X-ray absorptiometry(DXA).Density,according to WHO recommended diagnostic criteria,the study subjects were divided into osteoporosis group,low bone mass group,normal bone mass group,X-ray standing knees and KL imaging grade,KL grade of normal control group was 0 The serum concentrations of P1NP,?-CTX,N-MID and VITD in different bone mass groups were detected by enzyme-linked immunosorbent assay(ELISA).The data were collected and analyzed by SPSS17 software.ResultsThe bone mineral density of the hips in the osteoporosis group,the low bone mass group and the normal bone mass group corresponding to different KL grades were significantly different by one-way ANOVA(P<0.05).The LSD test showed normal bone mass and low bone mass.The hip bone density corresponding to the K-L2 level of the measurement group was significantly lower than that of the K-LO level(P<0.05).The K-L4 grade bone density corresponding to the normal bone mass group and the low bone mass group was significantly higher than that of the K-L2 grade.(P<0.05),the hip bone density of the two groups decreased first and then increased,ranking K-L4>K-L0>K-L3>K-L1>K-L2;bone The K-L2 hip bone density in the loose group was significantly lower than that in the K-LO level(P<0.05),and the K-L4 hip bone density was significantly lower than the K-L3 level(P<0.05).The osteoporosis group The hip bone density of KL grade decreased first,then increased,and then decreased.The rank of each grade was K-LO grade>K-L3 grade>K-L1 grade>K-L2 grade>K-L4 grade.P1NP and ?-CTX were significantly different in the osteoporosis group,the low bone mass group and the normal bone mass group.In the normal bone mass group and the low bone mass group,the K-L2 grade P1NP and BCTX were higher than the K-LO grade.There was a significant increase(P<0.05),and K-L4 P1NP was significantly lower than K-L2(P<0.05).The order of P1NP and?-CTX in the two groups was K-L2>K-L1>K-L3 grade>K-L4 grade;K-L2 grade P1NP and ?-CTX in the osteoporosis group were higher than K-LO grade(P<0.05),K-L4 grade P1NP and ?-CTX were higher than K-L3 grade(P<0.05);N-MID and VITD were not significantly different in each group(P>0.05).There were significant differences in serum P1NP and sputum-CTX serum levels in patients with postmenopausal osteoarthritis.The levels of serum P1NP and sputum-CTX in liver and kidney deficiency type were higher than those in qi stagnation and blood stasis type,cold dampness type and damp heat type.Significantly increased P<0.05;serum N-MID,VITD showed no significant difference in each group(P>0.05).ConclusionThe increase of subchondral bone metabolism is closely related to the pathogenesis of post-menopausal knee osteoarthritis.Serum P1NP and ?-CTX levels are important for the early diagnosis of postmenopausal knee osteoarthritis,progression of the disease and guiding anti-bone resorption therapy.The traditional Chinese medicine syndrome differentiation of post-menopausal knee osteoarthritis provides an objective basis.
