Diagnostic Value Of Abnormal Prothrombin In Primary Hepatocellular Carcinoma

Objectives:To compare the sensitivity and specificity of alpha-fetoprotein(AFP)and abnormal prothrombin(DCP)in the diagnosis of primary hepatocellular carcinoma(PHC),and to explore the clinical value of DCP in the diagnosis of PHC.Methods:From January 2017 to December 2018,the specimens of outpatients,inpatients and healthy patients in the second People's Hospital of Yancheng were divided into five groups:healthy group(35 cases),chronic hepatitis B group(31 cases),cirrhosis group(34 cases),metastatic hepatocellular carcinoma group(32 cases)and primary hepatocellular carcinoma group(85 cases).The subjects fasted after 22:00 p.m.before blood collection,and 3 ml of venous blood were collected the next morning.Serum AFP and DCP levels were measured by radioimmunoassay.The levels of AFP and DCP in each group were compared,and the sensitivity and specificity of AFP and DCP in the diagnosis of PHC were calculated by comparing the area under ROC-AUC curve between healthy group,chronic hepatitis B group,cirrhosis group and metastatic hepatocellular carcinoma group.Results:Compared with the average level of DCP in healthy group,the average level of DCP in cirrhosis group,metastatic hepatocellular carcinoma group and primary hepatocellular carcinoma group was significantly higher(P<0.05).There was no significant difference between chronic hepatitis B group and healthy group(P>0.05).Compared with PHC group,the average level of DCP in chronic hepatitis B group,cirrhosis group and metastatic hepatocellular carcinoma group was lower and the difference was statistically significant.(P<0.05).Compared with the average level of AFP in healthy group,the average level of AFP in chronic hepatitis B group,cirrhosis group,metastatic hepatocellular carcinoma group and primary hepatocellular carcinoma group was also significantly higher(P<0.05),while compared with PHC group,the average level of AFP in chronic hepatitis B group,cirrhosis group and metastatic hepatocellular carcinoma group was also significantly lower(P<0.05).In the ROC-AUC statistical model,the area under the AUC curve of DCP for PHC was larger than that of AFP,no matter in healthy group,chronic hepatitis B group,cirrhosis group or metastatic hepatocellular carcinoma group(P<0.05).Conclusions:The levels of AFP and DCP in patients with PHC increased significantly.DCP was superior to AFP in the diagnosis of PHC.
