Application Of Case Management In Social Support Of Patients With Mental Disorders

August 26,2016,the CPC Politburo held a meeting to consider and adopt the "healthy China 2030" plan outline.The outline puts forward that health is the inevitable requirement to promote the all-round development of human beings and the basic condition of economic and social development.Health includes good health and mental health.According to the latest data released by the Mental Health center of the China Center for Disease Control and Prevention,the total number of people with various mental illnesses in China has exceeded 100 million,including those with severe mental disorders exceeding 16 million.Rising data,an increasingly severe situation,require us to strengthen our focus on mental health.In the long-term illness of patients with mental disorders,doctors and primary caregivers pay more attention to the stability of the disease,while ignoring the psychological and social treatment,the patient’s social support system gradually weakened.Therefore,it is necessary for social workers to intervene in the treatment of patients with mental disorders,through the way of case management,to enhance the social support of patients.S city serious mental disorder Kinship Care Project is S City psychiatric hospital Guangji Hospital and Gu sur District rehabilitation station and other multi-party cooperation projects,Through the screening of pre-investigation,it is determined that the service target is a total of 26 patients with severe mental disorders and families in the district,The joint psychiatrist、rehabilitation station sperm prevention staff and social workers,among others,form a care group to provide case management services.Based on Maslow’s theory of need hierarchy、social support,this paper makes use of literature method、Questionnaire survey method and interview method.Using the practical model of case management to intervene in the study of social support promotion for patients with mental disorders,try to solve the main problem of the case through personalized service scheme.Through the assessment,it is found that after an average of one year,15 times an average annual intervention,the main social support ability of the case is significantly improved,set the goal is basically completed.However,the improvement of mental health policies、social enhancement of the tolerance of people with mental disorders and the increased professional competence of social workers acting as case managers continue to be a long and far-standing process in the market.
