Objective:To investigate the relationship of homocysteine(Hey)and its metabolic enzyme N5,N10-methylenetetrahydrofolate reductase(MTHFR)gene and intracranial atherosclerosis The relevance of Intracranial atherosclerotic stenosis(ICAS).Methods:91 patients with ischemic stroke were divided into stenosis group and non-stenosis group based on the presence or absence of intracranial blood vessels.The two groups were 37 and 54 respectively,based on enzyme circulation method and PCR-RFLP(polymerase The chain reaction-restriction fragment length polymorphism technique was used to measure and analyze the serum homocysteine concentration and MTHFR gene polymorphism in each group.The data processing was performed by SPSS 24.0 software.The X2 test was used to compare the data between the groups.The t-test was used to compare the data between the groups.The statistical significance was P<0.05.Result:(1)The concentration of Hey in the stenosis group was higher than that in the non-stenosis group(14.83±4.69 umol/1:10.30±3.85 umol/1;t=4.42,P<0.01);the stenosis group had higher Hey concentration than the non-stenosis group(15.66±5.26 umol/1:9.20±4.90 umol/1;t=2.86,P=0.01);the concentration of Hey in the stenosis group was significantly higher than that in the non-stenosis group,male or female,the difference was statistically significant(15.00±4.76 umol)./1:10.04±4.10 umol/1;t=5.32,P<0.01).(2)The Hey concentration(15.14±7.80 umol/1)of patients with homozygous mutant TT genotype was significantly higher than that of CC genotype patients(11.14±3.29 umol/1)and Hcy concentration of CT genotype patients(11.74±4.65 umol/1),and the difference was statistically significant.(3)There was no significant difference in the distribution of MTHFR genotypes(CC type,CT type and TT type)between the stenosis group and the non-stenosis group.(4)Logistic multivariate regression analysis indicated hypertension(OR=25.37 95%CI:3.25?198.10 P=0.002),diabetes(OR=7.96 95%CI:1.99?31.79 P=0.003)and Hcy(OR=1.44)95%CI:1.21 to 1.70 P=0.000)is an independent risk factor for ICAS.Conclusion:(1)Homocysteine concentration is a risk factor for intracranial atherosclerotic stenosis.(2)MTHFR gene polymorphism affects the concentration of serum Hcy.The serum Hcy concentration of TT patients is significantly higher than that of CC and CT genotypes.(3)MTHFR gene polymorphism is not associated with intracranial atherosclerotic stenosis.
