Objective:The purpose of this study was to compare the diagnostic efficacy of131I-MIBG radionuclide planar imaging,CT scan and131I-MIBG radionuclide planar imaging combined with SPECT/CT fusion imaging in the diagnosis of adrenal diseases,and the diagnostic value of131I-MIBG radionuclide planar imaging combined with SPECT/CT fusion imaging in the diagnosis of adrenal diseases was explored.Materials and Methods:The data of 41 patients who were suspected adrenal diseases and underwent 131I-MIBG SPECT/CT radionuclide imaging from 2012 to 2018 in the department of nuclear medicine,the first affiliated hospital of Chongqing Medical University were collected.All patients received surgical treatment after examinations and were finally confirmed by pathological biopsy.All the data were divided into pheochromocytoma group and non-pheochromocytoma group according to the pathological results.All subjects underwent 131I-MIBG radionuclide imaging,CT imaging,and 131I-MIBG SPECT fusion imaging.The ROC curves of whole body imaging score and lesion diameter were plotted to find the best cut-off value.The correlative factors affecting the results of general examination were analyzed.Results:The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of 131I-MIBG systemic imaging in the diagnosis of adrenal diseases were 90.5%,60.0%,75.6%,70.4%and85.7%,respectively.The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of CT scan in the diagnosis of adrenal diseases were 90.5%,20.0%,56.1%,54.3%and 66.7%,respectively.The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of 131I-MIBG combined with SPECT/CT fusion imaging in the diagnosis of adrenal diseases were 90.5%,75%,82.9%,79.2%and 88.2%,respectively.The specificity,accuracy and positive predictive value of 131I-MIBG SPECT/CT fusion image were higher than that of CT,and the difference was statistically significant?P<0.05?.When the value of lesion diameter was equal or higher than 3.55cm was taken as the boundary value,the youden index was the highest,and the diagnostic sensitivity and specificity were 68.2%and 94.7%respectively.When the whole body imaging score was greater than 3.5 points which means scored 13 was negative and scored 4 was positive as the boundary value,the youden index was the highest,and the diagnostic sensitivity and specificity were 86.4%and 89.5%respectively.The imaging results were significantly correlated with increased blood catecholamine and metabolites?P=0.026?and lesion diameter?P=0.000?.Conclusion:131I-MIBG radionuclide planar imaging combined with SPECT/CT fusion imaging has higher sensitivity,specificity,accuracy,positive predictive value and negative predictive value in the diagnosis of adrenal diseases compared with 131I-MIBG radionuclide planar imaging.131I-MIBG SPECT/CT fusion image is superior to CT in the diagnosis of adrenal diseases.131I-MIBG radionuclide planar imaging combined with SPECT/CT fusion imaging has irreplaceable advantages in the diagnosis of adrenal diseases.
