Objective To analyze the clinical value of nimodipine stress single-photon emission computed tomography perfusion imaging in acute non disabling cerebrovascular disease.Methods Thirty patients with acute non disabling cerebrovascular disease in the emergency hospital from Xiamen University First Affiliated Hospital from December 2016 to November 2017(22 males,8 females,average age 64.10±2.27 years who underwent 99Tcm-ECD and nimodipine stress CBF were enrolled.Regions of interests(ROIs)were drawn in the bilateral frontal lobe,temporal lobe,parietal lobe,occipital lobe,basal ganglia,cerebellum,brainstem,the ratio of regional cerebral blood flow(r CBF%)of all ROIs were calculated.The detection rate by post-and prenimodipine stress SPECT,MRI were calculated respectively.The reaction to nimodipine stress cerebral blood flow were observed.And analyzed using ?2 test.Results 1,The detection rate of MRI,rest and nimodipine stress SPECT was 46.67%(14/30)?73.33%(22/30)?and 90.0%(27/30).2,There were four types of cerebrovascular reactivity to nimodipine:the first,r CBF%was normal before nimodipine stress,and then decreased after the test in 2 patients;the sencond,c CBF%was decreased before the nimodipine stress,and there was no change after nimodipine stress in 5 patients;the third,c CBF%was decreased in the rest,and then further decreased after the text in 10 patients;and the forth,r CBF%was decreased before the nimodipine stress and then increased after the text in13 patients.3,Of the 30 cases,4 cases of SPECT imaging showed an inability to cross the cerebellar neurologic connection,showing a reduction in the contralateral cerebellar perfusion in a new cerebral hemisphere infarction,but no obvious abnormalities in the MRI cerebellum.4,The incidence of nimodipine load test in 30 cases was 6.67%(2/30),and 2 cases were mainly characterized by the decrease of blood pressure.After a few minutes of slow infusion,the blood pressure was gradually stable,and no discomfort such as panic and sweating were found.Conclusions 1,Compared with MRI,combined resting and nimodipine SPECT brain perfusion imaging can improve the diagnostic rate of acute non-disabling cerebrovascular disease.2,By comparing the changes of local cerebral blood flow before and after vasodilatation,there are 4 different types of cerebrovascular responses in Nimodipine load SPECT imaging,which can be used to evaluate the function of cerebral vascular reserve.3,Nimodipine stress SPECT is safe and effective.
