Objectives To observe the expression of regulatory T cells and cytokines TGF-beta,IL-35 and IL-10 in peripheral blood of patients with myelodysplastic syndrome(MDS)and aplastic anemia(AA)and to explore their clinical significance.Methods Thirty-six newly diagnosed patients with myelodysplastic syndrome from October 2017 to December 2018 in the Affiliated Hospital of North China University of Technology were selected as MDS group.Six patients with hypo-MDS and 24 patients with aplastic anemia were selected as AA group,including 8 SAA and 16 NSAA.The peripheral blood mononuclear cells were isolated from peripheral venous blood.The proportion of Treg cells in CD4~+T cells was detected by flow cytometry.The serum levels of TGF-beta,IL-35and IL-10 were measured by enzyme-linked immunosorbent assay.All data were input into SPSS 17.0 statistical software for statistical analysis.Normality test was carried out on the test indicators.Measured data conforming to normal distribution were expressed by(mean+standard deviation)(x?±s).T test was used for data comparison between the two groups.Measured data of non-normal distribution were expressed by M(P25,P75).Rank sum test was used for comparison between the two groups.The influence of MDS and AA was used for counting data.The factors were analyzed by unconditional binary logistic regression,and the test level was P<0.05 with statistical significance.Results 1 Compared with AA group,Treg cell proportion(4.39±1.23%),TGF-beta(87.32±67.63pg/ml),IL-35(114.19±65.69pg/ml),IL-10(51.82±23.38pg/ml)expression levels in MDS group were higher than those in AA group(2.45±0.42%),(31.68±11.95pg/ml),(32.53±11.44pg/ml)and(12.10±4.12pg/ml),with significant difference(P<0.05).The percentage of Treg cell(3.67±0.66%),TGF-beta(67.04±26.30pg/ml),IL-35(72.85±31.13pg/ml),IL-10(55.78±19.31pg/ml)in hypo-MDS were higher than those of AA group,with significant difference(P<0.05).2 The relationship between Treg cells,TGF-beta,IL-35 and IL-10 and the risk of MDS and AA was further analyzed by binary logistic regression analysis.The results suggest that IL-35 is a risk factor for MDS and the proportion of bone marrow lymphocyte is a risk factor for AA.3 MDS was divided into H-MDS and L-MDS according to IPSS-R score.Treg cells in peripheral blood of H-MDS group accounted for(4.92±1.24%)of CD4~+T lymphocyte,and the concentrations of TGF-beta(140.95±51.34pg/ml),IL-10(70.68±7.76pg/ml)were higher than those of L-MDS group(3.84±0.97%),(33.69±26.25pg/ml),(32.97±17.69pg/ml),P<0.05.Treg cells(4.85±1.31%),TGF-beta(128.46±56.93pg/ml),IL-10(68.85±11.48 pg/ml)in MDS patients with bone marrow primordial cell ratio>5%were higher than those in MDS patients with bone marrow primordial cell ratio<5%(3.79±0.82%),(35.89±38.57pg/ml),(30.54±15.48pg/ml),P<0.05.Treg cell percentage(2.62±0.41%)and IL-35(36.48±12.19 pg/ml)levels in NSAA group were higher than those in SAA(2.12±0.96%),(24.63±2.14pg/ml),with significant difference(P<0.05).Conclusions 1 The levels of Treg cells,TGF-beta,IL-35 and IL-10 in peripheral blood of MDS patients were higher than those of AA patients.2 Increased IL-35 level is a risk factor for MDS,and increased bone marrow lymphocyte ratio is a risk factor for AA.3 The expression of Treg cells in MDS and AA may be related to the severity of disease.Figure 0;Table 9;Reference 38.
