Significance Of Plasma Fibrinogen And Platelet Levels In Elderly Patients With Inoperable NSCLC

Abstract/Summary:
Objectives To investigate the level and significance of plasma fibrinogen(FIB)and platelet(PLT)in elderly patients with inoperable NSCLC.Methods1 Data of 165 patients who without anytreatment,over 65 years old,were assessed to be unable to undergo surgery,were initially diagnosed by pathology in Hebei Provincial People’s Hospital,from January 2013 to December 2017 were collected.In addition,blood routine,coagulation function and biochemical indicators were collected from 50 elderly health examinees.2 According to median of patient data literature at home and abroad,the boundaries of FIB and PLT were determined to be 4 g/L and 400×109/L respectively,and grouped into groups.3 The levels of FIB and PLT in elderly patients with inoperable NSCLC were compared with those in healthy people,and the relationship between the levels of FIB and PLT and clinicopathological features and prognosis was analyzed.Results1 Comparison of FIB and PLT levels in peripheral blood of inoperable elderly patients with NSCLC and healthy elderly peopleThe levels of FIB and PLT in peripheral blood of the inoperable elderly patient swith NSCLC were 4.16±1.68 g/L、314.20±103.94×109/L,respectively;the levels o f FIB and PLT in the healthy elderly were 3.19±1.42 g/L、262.93±74.54×109/L,re spectively;the levels of FIB and PLT in the NSCLC group were significantly high er than those in the healthy group(t1=7.454,P1<0.05,t2=7.317,P2<0.05).2 The relationship between FIB and PLT levels with the clinicopathologic features at diagnosis.FIB and PLT levels in elderly patients with inoperable NSCLC were not correlated with age,sex,smoking index,body mass index(BMI),underlying diseases and pathological types(P>0.05);positively correlated with the ECOG score,Distant metastasis,pathological differentiation and clinical stage;negatively correlated with hemoglobin(HB)and serum albumin(ALB)(P<0.05).3 The relationship between FIB,PLT levels and prognosis at diagnosisThe 165 patients’ median survival time(MST):9 months;Those survival rates of six months,1、2 and 3 years were respectively:70.30%,43.03%,10.90%and 3.63%.Univariate analysis showed that Overall survival time(OS)in 165 elderly patients with NSCLC was not related to gender,smoking index,BMI,complications or pathological types(P>0.05),but related to age,distant metastasis,differentiation degree,clinical stage,FIB,PLT,HB and ALB level(P<0.05).There were 75 cases in FIB>4g/L group,the OS was 35 months,MST was 5 months;There were 90 cases in FIB<4g/L group,the OS was 72 months,MST was 12 months.Those survival rates of 1、2 and 3 year were respectively 17.33%,1.33%,1.33%,64.44%,18.88%and 6.66%,there was a statistically significant(x2=53.996,P<0.05).There were 54 cases in PLT>400×109/Lgroup,total OS was 21 months,MST was 4 months;There were 111 cases in PLT<400×109/Lgroup,total OS was 72months,MST was 11 months.Those survival rates of 1、2 and 3 year were respec tively 5.55%,1.20%,0.00%,61.26%,16.21%and 5.40%.there was a statistically s ignificant(x2=122.720,P<0.05).COX multivariate analysis showed that FIB and PLT levels(B1:0.554,HR1:1.704,95%CI1:1.711-2.711;B2:1.403,HR1:4.068,95%CI2:2.363-7.002)were independent risk factors for the prognosis of inoperable elderly NSCLC(P<0.05).Conclusions1 The levels of FIB and PLT in peripheral blood of inoperable elderly NSCLC were higher than those of elderly healthy people.2 The levels of FIB and PLT were related to age,physical condition,nutritional status,pathological differentiation and clinical stage.3 The higher level of FIB and PLT,the worse the prognosis of elderly NSCLC.Figure 10;Table 14;Reference 49...
