Short-term Effects Of Hemodialysis On Hearing Function In Patients With Uremia

Objective:The data of pure tone audiometry and distortion product otoacoustic emission related heraing function were collected one hour before hemodialysis,one hour after hemodialysis and three days after hemodialysis.The short-term effects of hemodialysis on the hearing function of patients with uremia were analyzed.Methods:Twenty-five uremic patients(50 ears)who were admitted to the blood purification center of Weifang People's Hospital from June 2018 to December 2018 were randomly selected as the research objects.One hour before,one hour after and three days after the certain hemodialysis,pure tone audiometry and distortion product otoacoustic emission were performed on the subjects.The three pure tone audiometry thresholds at 0.25,0.5,1.0,2.0,4.0 and 8.0KHz were recorded respectively.The amplitudes and passing rate of the three distortion product otoacoustic emission at 0.5,0.7,1.0,2.0,3.0,4.0,6.0 and 8.0KHz were recorded respectively too.Results:1.The average thresholds of pure tone audiometry at 0.25,0.5 and 1.0KHz one hour before hemodialysis were recorded as(26.67±9.55)dB,(24.72±7.17)dB,(23.61±7.03)dB respectively,and one hour after hemodialysis the average thresholds of pure tone audiometry at 0.25,0.5,1.0 KHz were recorded respectively as(22.22±6.47)dB?(21.39±4.47)dB?(21.39±5.64)dB.The hearing threshold one hour before hemodialysis at 0.25,0.5 and1.0KHz was higher than the corresponding frequency hearing threshold one hour after hemodialysis,the difference between the comparison groups was statistically significant(P<0.05).The other frequencies were statistically analyzed,and the difference between the comparison groups was not obvious(P>0.05);On the third day after the end of hemodialysis,no significant difference was found in the threshold of pure tone audiometry at 0.25,0.5 and1.0KHz compared with the previous two data.2.The amplitude of the distortion product otoacoustic emission at 0.7,1.0 KHz one hour before hemodialysis is respectively(-1.39±7.15dB)dB SPL?(-4.39±12.19)dB SPL,and amplitude of the distortion product otoacoustic emission one hour after hemodialysis is respectively(3.28±7.72)dB SPL?(1.33±8.17)dB SPL.The reaction amplitude at 0.7,1.0 KHz is significantly improved in hemodialysis for one hour.The difference between the comparison group was statistically significant(P<0.05).The amplitude of the other frequencies was not significantly changed by statistical analysis,and the difference between the control groups was not significant(P>0.05).On the third day after the end of hemodialysis,there was no significant difference in the threshold of otoacoustic emission at 0.7 and 1.0KHz compared with the previous two data.3.One hour after hemodialysis,the passing rate of otoacoustic emission at 0.7 KHz was significantly improved,and the difference between the comparison groups was statistically significant(?~2=6.061,P<0.05).The other frequencies were not significantly changed by statistical analysis,and the difference between the comparison groups was not obvious(P>0.05).Conclusions:Hemodialysis has a positive effect on the low frequency hearing function of patients with uremia in a short term.The results show that after hemodialysis,the low frequency threshold of the pure tone(0.25,0.5,1.0 kHz)becomes smaller,and the low frequency response amplitude of the distorted product otoacoustic emission(0.7,1.0 kHz)increases.With the end of hemodialysis,the status of low-frequency auditory function of patients deteriorated again,but did not reach the status before hemodialysis.Hemodialysis had no significant effect on the patients'high-frequency hearing function in the short term.
