[Objective]Primary hepatocellular carcinoma is one of the most common tumors in the digestive system.Its incidence rate ranks fifth,and its mortality rate ranks third.In our country,the annual mortality rate of this disease ranks second in cancer mortality.The age of onset of liver cancer patients is mostly 40 ? 50 years old,male more than female.Due to its high degree of malignancy,hepatocellular carcinoma is not sensitive to radiotherapy and chemotherapy.Early diagnosis and treatment(combined treatment based on surgical resection)is the key to improving the long-term therapeutic effect of hepatocellular carcinoma.Influence on the occurrence,development and prognosis of liver cancer is various,C1QL1 gene is a member of the C1 q protein family,is C1QL1 C1q-like a small secreted proteins,almost fully expressed in the brain,C1q-like(C1QL)-1,2,and 3 proteins are made up of homologous gene encoding highly expressed in the brain,studies have shown that the present C1QL1 has been found in the excessive expression in colorectal cancer could be as a marker of colorectal cancer prognosis,Studies have also shown that C1QL1 is in differentiated thyroid cancer.C1QL1,which is different from normal thyroid tissue,can be used as one of the biomarkers of differentiated thyroid cancer,and predicted with the CRABP1 and LCN2 differentiated thyroid cancer spread outside the thyroid.The diagnosis and prognosis of thyroid cancer have important clinical significance,but C1QL1 expression in HCC and its clinical significance is still remains to be studied,this research adopts the immunohistochemical detection technology,Western blotting detection and qRT-PCR experiments C1QL1 expression in HCC tissues,The relationship between its high expression and the occurrence,progression,malignancy and clinicopathology of HCC was also discussed.[Methods] The expression of C1QL1 protein in 80 cases of hepatocellular carcinoma was detected by immunohistochemistry.The expression of C1QL1 protein in frozen tissues and 20 C1QL1 protein expression in cancer tissues were detected by qRT-PCR and Western blot method.The expression level of C1QL1 in paraffin-embedded tissues of liver cancer and the related clinicopathological data and prognosis were analyzed.[Results] Immunohistochemical results of 80 paraffin tissue samples showed that C1QL1 protein was highly expressed in 55(68.8%)HCC tissues.The expression of C1QL1 in HCC was correlated with the presence or absence of history of hepatitis b(P= 0.021),tumor size(P= 0.003),vascular invasion(P= 0.003),TNM stage(P=0.002)and Edmondson grade(P=0.002).It was not related to age(P= 0.758),gender(P=0.672),alpha fetoprotein(AFP)(P=0.191),number of tumors(P=0.877),child-pugh grade(P=0.866),tumor envelope integrity(P=0.238)or liver cirrhosis(P=0.316).Both qrt-pcr and Western blot showed that the expression level of C1QL1 in HCC tissues was significantly higher than that in the corresponding paracancer tissues,and the difference between the two groups was statistically significant(P < 0.01,P < 0.001).Among the 80 patients with HCC,55 patients had high expression of C1QL1,25 patients had low expression,and 8 patients were lost to follow-up(5 patients with high expression of C1QL1 and 3 patients with low expression of C1QL1).Kaplan-meier survival analysis showed that the median survival period of the patients with high expression was 17 months,while that of the patients with low expression was 33 months.Log-rank test showed that the survival rate of the patients with high expression of C1QL1 protein was significantly different from that of the patients with low expression(P < 0.05).[Conclusion]C1QL1 protein may play a positive up-regulation role in the occurrence,development and malignancy of HCC,suggesting that the high expression of C1QL1 protein may be related to the occurrence,development and malignancy of HCC,and may become a new biomarker for early diagnosis of HCC and a new target for clinical molecular targeted therapy.
