Comparison Of Safety And Efficacy Between Low Power And High Power Holmium Laser Enucleation Of The Prostate

ObjectiveHolmium laser enucleation of the prostate(HoLEP)was considered as an ideal way for the treatment of BPH.Traditionally,HoLEP was performed using high power holmium laser devices,which restricted the application of HoLEP.Though some research showed the possibility of HoLEP with low power holmium laser,there was less research about the direct contrast between low power HoLEP(LP-HoLEP)and high power HoLEP(HP-HoLEP).To assess whether LP-HoLEP could behave as good as HP-HoLEP,we compared the safety and efficacy between LP-HoLEP and HPHoLEP.Patients and MethodsFrom September 2016 to March 2018,a total of 120 BPH patients in our hospital underwent HoLEP,with 65 watt for LP-HoLEP(60 patients)and 100 watt for HPHoLEP(60 patients).Enucleate prostate weight,enucleation efficiency,operation time,postoperative bladder irrigation time,indwelling catheterization time,hospital stays and operative complications were recorded.Qmax,PVR,IPSS and QoL were evaluated at 1,3 and 12 months after surgery.ResultsAll patients underwent successful HoLEP and no significant complication happened.Enucleate prostate weight,enucleation efficiency,operation time,postoperative bladder irrigation time,indwelling catheterization time,hospital stays and operative complications showed no statistically significant differences(P>0.05).Qmax?PVR?IPSS and QoL were significant improved at 1,3 and 12 months(P<0.05),and there was no significant difference between two groups(P>0.05).ConclusionLP-HoLEP is a safe and effective procedure for the treatment of BPH.It is noninferior to HP-HoLEP in terms of enucleate prostate weight,enucleation efficiency,operation time,postoperative bladder irrigation time,indwelling catheterization time,hospital stays and operative complications.LP-HoLEP could alleviate LUTS and improve quality of life as good as HP-HoLEP.LP-HoLEP is cost-effective and worthy of clinical promotion.
