Objective: To investigate the relationship between the ratio of neutrophil to lymphocyte and the degree of coronary artery disease and left ventricular function in elderly patients with myocardial infarction Methods: The aim of this study was to investigate the relationship between the degree of coronary artery disease and the ratio of neutrophil to lymphocyte NLR in STEMI patients.From November 2013 to November 2018,the Department of Cardiology of the Second Affiliated Hospital of Tianjin Medical University was concurrently crowned.There were 238 patients with STEMI who underwent angiography,including 131 male patients and 107 female patients.The included patients met the STEMI diagnostic criteria,namely ST-segment elevation and myocardial enzyme changes,and excluded non-conformities.The patient’s general clinical and laboratory data were collected and patients were divided into high-group(>90 points)75 cases,medium group(60-90 points)75 cases,and low group(≤60 points)88 cases according to Gensini scores.According to the number of vascular lesions,50 patients were divided into single-vessel disease group.The double-vessel disease group had 2 lesions,45 patients,2 left-sided lesions,and 143 patients with three-vessel disease.The differences in general clinical data between the high,middle and low groups were compared.The NLR values of the three groups with different coronary lesions,the three groups with different coronary lesions,and the different gender groups were compared.There is no difference.Finally,the correlation between the value of NLR and Gensini score is studied.Statistical software SPSS is used for statistical analysis.The measurement data is expressed in the form of mean±standard deviation(`x±s),and the count data is percentage.form.The measurement data satisfying the normal distribution was analyzed by variance analysis to analyze the differences between groups.The measurement data that did not satisfy the normal distribution were analyzed by rank sum test to analyze the differences between groups.The single factor analysis was analyzed by one-way analysis of variance,and the correlation was consistent with the binary normal distribution.The variables are linearly correlated,and the rank correlation analysis is performed on variables that do not conform to the binary normal distribution,and a scatter plot is drawn.The difference was statistically significant at P < 0.05.The severity of coronary lesions was scored using the Gensini integration method.The results showed that: 1.There were significant differences in cholesterol,neutrophil percentage,lymphocyte percentage and age between the high,middle and low groups(P<0.05).The two pairs were found to be high.The difference between the group and the low group was statistically significant(P < 0.05).The age is statistically significant between the high and low groups and the high and medium groups.The percentage of lymphocytes also had a statistically significant difference between the three groups(P<0.01).The difference between the percentage of lymphocytes in the low group and the high group was statistically significant(P<0.01),which was different from the middle group.However,it was not significant(P<0.05),and the difference between the high group and the LYM% of the middle group was not statistically significant.Other clinical data were not statistically different between the three groups.2.There was a statistically significant difference in NLR between the three groups(P<0.05).See Table 6,where the difference in NLR between the high and middle groups was statistically significant.(P < 0.05),and the difference between the high group and the middle group and the middle group and the low group was not statistically significant(P > 0.05).There was no significant difference in NLR between single-vessel disease group and multi-vessel disease group(P>0.05).There was a statistically significant difference in NLR between male and female patients(P<0.01).The NLR value of male patients was significantly higher than that of females,but the degree of coronary lesions was not significantly different(P>0.05).The rank correlation analysis of NLR and coronary lesions in STEMI patients was performed by SPSS software.The results are shown in Fig.1.The results show that there is a positive correlation between them(r=0.251 P=0.000<0.01),and the correlation is significant.The following conclusions are drawn:1.NLR is closely related to the degree of coronary artery disease in STEMI patients,and has a positive correlation,that is,the degree of coronary lesions increases with the increase of NLR.2.There is no significant correlation between the value of NLR and the number of coronary lesions in STEMI patients,which may be due to the small amount of data in this study.3.The NLR value of male patients in STEMI patients was significantly higher than that in females,indicating that the degree of inflammatory response in different gender patients was affected by factors such as sex hormones.4.NLR can be used as a preliminary screening factor for myocardial infarction,predict disease risk,and also help to determine the condition of patients who cannot be angiographic for various reasons.
