The Correlation Between Renal Anemia And Constitution Of Traditional Chinese Medicine In Patients With Chronic Kidney Disease Stage 4

Background:Chronic kidney disease(CKD)is the common outcome of various kidney disease progression.There’s about 10.8% of adults suffer from CKD in China,while the awareness rate is only 12.5%.Renal anemia is a common complication of CKD,which can aggravate the damage of multiple systems and organs.It is an independent risk factor for patients to complicate with cardiovascular complications.Anaemia can accelerate kidney function in turn,and reduce the life quality of the patient.Although modern medicine can improve anemia,but it is difficult to maintain a relatively stable level of hemoglobin(Hb).Fluctuations in hemoglobin can worsen renal function and increase the risk of cardiovascular events."TCM constitution" is the treasure of ancient Chinese medicine.According to the principle of "TCM constitution can be classified and regulated,constitution is related to disease",understanding the constitution distribution of patients with renal anemia is expected to provide a broader vision for the treatment.Objective:To research the TCM constitution characteristics of patients with renal anemia in stage 4 CKD,and to understand the predisposing constitution of renal anemia.To research the correlation between TCM constitution and relevant clinical indicators in patients with renal anemia in stage 4 CKD.Methods:A total of 88 patients who met the inclusion criteria in the nephrology department of the affiliated hospital of chengdu university of traditional Chinese medicine from March 2018 to February 2019 were collected.Patients were asked to fill in the questionnaire named "TCM constitution classification and judgment".The researchers recorded the personal information(Name,sex,age,body mass index,smoking,sleeping time)and the relevant examination data(Hemoglobin,Serum creatinine,Blood urea nitrogen,uric acid,cystatin-C,cholesterol,triglyceride,HDL-c,LDL-c)of the patients,then used SPSS19.0 software to analysis the data and compare the differences of relevant clinical indicators in each group.Results:76 patients(86.4%)had renal anemia among the 88 patients with CKDstage 4,and 12(13.6%)had normal hemoglobin.The degree of anemia is mainly mild anemia.The smoking rate and BMI of the anemia group were higher than those of the non-anemia group,and the sleeping time was less than those of the non-anemia group.The differences in smoking,BMI,and sleeping time between the anemia group and the non-anemia group were statistically significant(P<0.05).Patients with TCM constitutions of qi deficiency,Yang deficiency,Yin deficiency,phlegm-dampness and blood stasis showed statistically significant differences in hemoglobin values(P<0.05).The TCM constitution distribution of 76 patients with renal anemia was: qi deficiency>Yang deficiency>phlegm and dampness>Yin deficiency>blood stasis> qi stagnation>damp heat>and mild heat > essential quality.Conclusion:The constitution type of patients with CKD4 stage is mainly asthenia,followed by parenchyma.In asthenia,qi asthenia is more common,and phlegm-dampness is more common in parenchyma.patients with qi asthenia are prone to renal anemia.
