Objective To explore whether the use of opioids and antiemetics and the reduction of pain scores(Numeric Rating Scales,NRS)in the abdominal surgery of the PACU(Postanesthesia Care Unit)through ultrasound-guided transversus abdominis plane block(TAP block)can be used as an assistant in analgesia after abdominal surgery.Methods This was a single-center retrospective study.Included patients who underwent abdominal surgery in our hospital between 1 October 2016 and 31 December 2016 and were subsequently sent to PACU for resuscitation.The total number of patients was 1045.Surgery time,ASA classification,gender,age,weight,surgical method,operation time,PACU time,analgesics,antiemetics and pain scores were recorded in group TAP and group NTAP.The main comparison data were as follows: general condition comparison between group TAP and group NTAP;The factors influencing pain scores and opioid supplementation were analyzed.Comparison of antiemetics between group TAP and group NTAP.Results A total number of 1045 patients with abdominal surgery were recorded in this study.Among them,363 patients received TAP block,and 682 patients did not receive TAP block.(1)comparison of general conditions of patients: There were no significant difference in gender,age,body weight and operation time in group TAP and group NTAP,The PACU time in group TAP was lower than that in group NTAP,and the difference was statistically significant(P < 0.001).(2)Analysis of impact pain scores and additional factors of opioids: gender,age and surgery Methods and operation time were statistically significant in the regression analysis(P < 0.05),among which male,elderly,laparoscopic operation and shortened operation time were the protective factors to reduce pain scores.TAP block in laparoscopic operation could not significantly reduce the pain scores.TAP block in patients undergoing open operation could reduce pain scores,and the difference was statistically significant(P < 0.05).No statistically significant difference was found in the use of opioids in PACU between laparoscopic surgery and open surgery in group TAP and group NTAP.(3)TAP block on antiemetics comparison: There was no significant difference in the use of antiemetics between the group TAP and the group NTAP during laparoscopic surgery(P>0.05).In the open surgery,the group TAP used antiemetics(9%)was lower than the group NTAP(21%).The difference was statistically significant(P < 0.05).Conclusion TAP block,as an adjuvant to postoperative analgesia,can reduce the pain scores of patients undergoing open surgery and reduce the use of antiemetics,but cannot reduce the use of opioids.
