Quadratus Lumborum Block Versus Transversus Abdominis Plane Block For Postoperative Analgesia In Patients Undergoing Abdominal Surgeries: A Meta-analysis

Objective:The aim of this study is to conduct a systematic evaluation of the existing clinical controlled trials and thereby understand the efficacy and safety of quadratus lumborum block and transversus abdominis plane block for pain management after abdominal surgery.Methods:We comprehensively searched PubMed,Embase,EBSCO,the Cochrane Library,Web of Science,WanFang,VIP and CNKI for randomized controlled trials(RCTs)that compared QL block and TAP block for pain management in patients undergoing abdominal surgery.All of the data were screened and evaluated by two researchers.RevMan5.3 was applied to perform the meta-analysis.Results:A total of 14 RCTs involving 952 patients(QLB=475,TAPB=477)were included and the results were as follows1.There was significant difference in VAS scores between the two groups at 2h,6h and 12 h after operation(P<0.05).VAS scores in QLB group were better than that in TAPB group;There was significant difference in postoperative 12 h VAS at rest or activity between the two groups(P<0.05).VAS scores in QLB group were better than that in TAPB group;There was no significant difference in postoperative 24 h VAS at rest or activity between the two groups(P>0.05).2.The amount of postoperative opioids consumption was lower in the QLB group than in the TAPB group,and the difference was statistically significant(P<0.05).3.There was no statistically significant difference between the two groups with regard to the postoperative analgesic satisfaction(P>0.05).4.Compared with the TAPB group,the QLB group had a lower incidence of postoperative nausea and vomiting,and the difference was statistically significant(P<0.05).Conclusion:Compared with TAPB,QLB can significantly relief the short-term postoperative pain,decrease the postoperative opioid consumption,and reduce the incidence of PONV.However,the long-term analgesic effects of the two groups may be the same,which still needs to be confirmed by clinical trials with a large sample size.
