Objective cavernous sinus invasion of pituitary adenomas could be the cause of difficulty in complete removal of the tumor,failure of hormonal remission and high reccrance.This study was designed to evaluate a magnetic resonance imaging criteria for cavernous sinus(CS)invasion in preoperative evaluation of pituitary adenomas named “periarterial enhancement”,the entire rimenhancement around the internal carotid artery.Expecting to improve surgical morbidity,treatment planning,and the prognosis for pituitary tumors.Methods: detailed database of 71 patients from March 2018 to December 2018 who underwent surgery for pituitary adenomas were retrospectively reviewed.Including the sellar MRIs,surgical records,operation videos,and the biopsy of sella turcica basement dura in pathological reports.The number of incomplete periarterial enhancement was calculated and assessed for its influence in the diagnosis of CS invasion.Results: among the 71 cases,16 of them had complete periarterial enhancement(22.54%),13 cases were proved having no invasion during operations(81.25%).Relatively,55 cases had incomplete periarterial enhancement(77.46%),46 of these cases included CS invasion during operations.(83.64%),The possiblity of intraoperatively confirmed invasions increased as increasing number of incompleteness periarterial enhancement was shown in every single case.(P<0.05)Conclusion: the number of incompleteness periarterial enhancement was highly related to the CS invasion of pituitary adenomas.The possiblity of CS invasions increased with the number of incompleteness periarterial enhancement increasing.
