| Objectives By comparing with furazolidone quadruple scheme different treatment of Helicobacter pylori(Helicobacter pylori,Hp)infection eradication therapy effect,understanding with furazolidone quadruple therapy for 10 days course of the eradication of curative effect,security and its related influencing factors,so as to improve the eradication rate and reduce the cost and its side effects,provide new ideas for Helicobacter pylori eradication therapy.Methods 420 Hp-positive patients diagnosed by 14C-UBT were randomly divided into two groups.240 patients in the experimental group were given rabeprazole 20 mg,amoxicillin 1g,furazolidone 100 mg and bismuth potassium citrate 0.6g twice a day for10 days.180 patients in the control group were given the same quadruple therapy for 14 days.Two groups of patients need to review the 14 C breath test after 4 weeks of withdrawal to determine whether Hp is eradicated or not.The clinical symptom relief score before treatment,after treatment and at the time of review is used to record the adverse reactions in the course of drug treatment.The difference of eradication rate,incidence of adverse reactions and degree of remission of clinical symptoms between the two groups was compared,and the related risk factors affecting Hp eradication effect were analyzed by binary logistic regression(P<0.05).Results 1 General situation: There were no significant differences in gender,age,body mass index,smoking and alcohol consumption between the two groups(P>0.05).2 Hp eradication rate: According to ITT analysis,the eradication rates of the two groups were83.8% in the experimental group and 84.4% in the control group.There was no statistical difference in the eradication rate between the two groups(χ2=0.037,P=0.847);according to the conformity scheme set(PP),the eradication rates of the two groups were 90.5% in the experimental group and 91.6% in the control group,respectively,and there was no statistical difference between the two groups(χ2=0.122,P=0.277).3 The degree of remission of clinical symptoms: The results of repeated measurement variance analysis showed that there was significant difference in time main effect(P=0.000),while there was no significant difference in treatment main effect(P=0.205),and there was no significant difference in the interaction between time effect and inter-group effect(P=0.588).4 Incidence of adverse reactions: There were 41 cases of adverse reactions in the two groups,including 17 cases in the experimental group and 24 cases in the control group.The incidence of adverse reactions in the experimental group and the control group were 8.29% and 16.90%,respectively.The difference was statistically significant(χ2=4.648,P=0.031).5 Hp related risk factors analysis: 1)Univariate analysis: gender had significant statistical effect on Hp eradication(P=0.019),age There was a statistically significant difference in the effect of Hp eradication(P=0.047),smoking There was a statistically significant difference in the effect of Hp eradication(P=0.003).There was no statistically significant difference in the effect of alcohol consumption on Hp eradication(P=0.013).There was no statistically significant difference in the effect of disease on Hp eradication(P=0.073).There was no statistically significant difference in the effect of Hp eradication(P=0.926).There was a statistically significant difference in the effect of body mass index on Hp eradication(P=0.006).The effect of expiratory load on Hp eradication was statistically different(P=0.000).2)Binary unconditional logistic regression analysis: gender(P=0.049,OR=0.245),smoking(P=0.030,OR=5.183),body mass index(P=0.031,OR=5.184),expiratory load(P=0.019,OR=6.715)is a risk factoraffecting the eradication of Helicobacter pylori.Binary logistic regression analysis of the10-day treatment course group and the 14-day treatment course group showed that there was no statistical difference in the curative effect between the two groups,indicating that the 10-day treatment course and the 14-day treatment course had the same effect on the eradication treatment.Conclusions 1 There was no significant difference in Hp eradication rate and clinical symptom remission between the 10-day treatment and the 14-day treatment in the 4-combination regimen containing furazolidone,but the incidence of adverse reactions of the former was significantly lower than that of the latter.2 Female,smoking,overweight and obesity,high expiratory load were the influencing factors of helicobacter pylori eradication.Figure 1;Table 9;Reference 146... |
