Applied Research On Food Exchange-based Dietary Management Among Patients With Severe Burn

ObjectiveTo explore the effects of food exchange-based dietary management on nutritional status,wound healing time,nutritional knowledge level of patients with severe burn in order to provide guidelines for effective dietary management among those patients.MethodsThis is a quasi-experimental study.According to the inclusion and exclusion criteria,70 patients with severe burn hospitalized in the Department of Burns and Plastic Surgery in an upper first-class hospital in Qingdao from April 2017 to September 2018 were included as subjects.35 cases enrolled from April 2017 to December 2017 were in the control group and 35 cases enrolled from January 2018 to September 2018 were in the experimental group.Thecontrol arm received a regular diet and The experimental arm received a foodexchange-based meal plan.Using expert consultation,we formulated our own nutritional knowledge questionnaire and tested its reliability and validity.Levels of biochemical parameters,such as serum prealbumin(PAB),serum albumin,serum total protein,and hemoglobin,in the two groups on admissionand 3 weeks after intervention were evaluated.Wound healing time of the two groups were recorded.The nutritional knowledge level of the two groups before and after interventionwere assessed.Statistical analyses were conducted using SPSS 25.0 to evaluate the effects of food exchange-based dietary managementin severe burn patients.Results 1 Finally,35 individuals in the experimental group and 35 in the control group completed this study.Before intervention,there were no statistical differences in the baseline data between the two groups(P>0.05).2 The reliability and validity test of the nutritional knowledge questionnaire for patients with burns showed that Cronbach's ?,I-CVI,S-CVI/Ave were 0.856,0.80,0.93,respectively.3After 3 weeks of intervention,the comparison between the two groups showed that the serum prealbumin level in the experimental group was higher than that in the control group(p<0.05),and the results of serum albumin,serum total protein and hemoglobin in the two groups were not statistically significant(p>0.05).After the intervention,the wound healing time of the experimental group was shorter than that of the control group(P < 0.01).At discharge,the nutritional knowledge score of the experimental group(21.29 ±2.27)was higher than that of the control group(16.97 ±1.07),and the difference was statistically significant(P < 0.01).ConclusionThis study conducted dietary intervention in patients with severe burns based on food-exchange method.As a result of our dietary management,the patients had improved nutritional status,effectively shortened wound healing time.Through nutrition knowledge education,elevated patients' knowledge level,and improved self-management capabilities.
