Purpose This paper describes the nutritional status of acute leukemia patients in China and the knowledge,attitude and behavior status of nutrition management of hematology medical staff,so as to provide basis for formulating optimal nutrition management strategies for adult acute leukemia patients.Methods PG-SGA,anthropometric indicators,laboratory indicators and Food Frequency Questionnaires were used to describe the nutritional status of 135 cases of acute leukemia patients,and descriptive studies were used to explore the obstacles of dietary intake during hospitalization of acute leukemia patients.A cross-sectional survey was conducted to investigate the knowledge attitudes and behaviors of 277 hematologists and nurses in nutrition management of adult acute leukemia patients.Results 1.49.63% of acute leukemia patients had malnutrition at admission,the incidence of malnutrition in ALL,AML and APL patients was 50%,53.97% and 40.63%,respectively,and the incidence of malnutrition in induced remission patients and consolidated maintenance patients was 55.29% and 40%,respectively.2.The incidence of malnutrition increased to 85.93% when acute leukemia patients were discharged from hospital,and the body mass index,serum albumin level and pre-albumin level were all significantly decreased,but only 5.97% of the patients received nutritional treatment.3.During the hospitalization of acute leukemia patients,the intake of all kinds of diets,except eggs and milk,was significantly reduced.The main factors hindering dietary intake of acute leukemia patients during hospitalization include treatment-related toxic and side effects,dietary restrictions of treatment,lack of nutritional knowledge of patients,dietary restrictions of basic diseases and lack of clear dietary guidance from medical staff.4.Through screening of relevant nutritional guidelines for cancer in recent five years at home and abroad and evaluation of methodological quality,the results suggest that the ?ESPEN guidelines on nutrition in cancer patients?are of the best quality.5.Guided by the knowledge-attitude-behavior theory and based on the ? ESPEN guidelines on nutrition in cancer patients?,we constructed ?Questionnaire on the knowledge,attitude and behavior of medical staff on nutrition management of cancer patients?,which has good reliability and validity.The internal consistency coefficient in the knowledge,attitude and behavior dimension were 0.859,0.814 and 0.876,the average Content Validity Index was 0.998.6.The accuracy of hematology medical staff in the knowledge dimension of nutrition management for adult acute leukemia patients was 39.64%,75.50% in the attitude dimension,and 72.12% in the behavior dimension.The "scores of questionnaire knowledge dimension","scores of questionnaire attitude dimension","doctor" and "participated in cancer nutrition related training" were protective factors for the nutritional management of adult acute leukemia patients by hematology medical staff.Conclusion 1.The incidence of malnutrition in acute leukemia patients in China is relatively high.Nearly 50% of the patients were malnourished when they were admitted to hospital.2.During the hospitalization and chemotherapy for acute leukemia patients,dietary intake was significantly reduced,and the reduction of dietary intake had a negative impact on the nutritional status of patients.The main factors hindering patients' dietary intake include treatment-related toxic and side effects,dietary restrictions on treatment,patients' lack of nutritional knowledge,dietary restrictions on basic diseases,and medical staff's lack of clear dietary guidance for patients.3.Hematology medical staff are not optimistic about the nutrition management of leukemia patients,which is mainly reflected in their low level of cancer nutrition knowledge,unreasonable attitude and improper behavior towards nutrition management of cancer patients.
