Objective1.Analysis of risk factors of deep venous thrombosis in neurosurgery: to explore the risk factors of DVT(Deep venous thrombosis)after neurosurgery,and to provide guidance for clinical nursing and prevention.2.Study on the effect of nursing intervention measures after meningioma operation: to explore the effect of nursing intervention acupoint stimulation on DVT in patients with meningioma after operation,and to provide evidence for clinical practice.Methods1.Analysis of risk factors of deep venous thrombosis in neurosurgery: hospitalized patients who were discharged from hospital in Qingdao,Shandong Province from January 2013 to January 2018 were selected to study by a matched case-control.The patients with DVT after operation were treated as DVT group.The patients with the same diagnosis,the same operation,the same sex and the age difference of(±3)years in neurosurgery at the same time were selected as non-DVT group.A case-control study was conducted at 1:2.All patients were excluded from lower extremity DVT,by color Doppler ultrasound before operation and had no history of DVT before.The general data of patients,disease-related data,treatment measures,related biochemical indexes,color Doppler ultrasound results and other medical records were collected.The statistical analysis was carried out by SPSS 25.0 software.Through univariate analysis and multivariate Logistic regression analysis,the statistically significant variables were screened out,and the statistically significant variables were selected.The OR value of correlation intensity with neurosurgical DVT and 95% confidence interval of statistically significant factors were calculated.To explore the independent risk factors of deep venous thrombosis in neurosurgery.2.Study on the effect of postoperative nursing intervention measures for meningeal tumors: from March 2018 to November 2018,148 patients with meningioma met the inclusion criteria in the neurosurgery inpatients of a third grade A hospital in Qingdao,Shandong Province.The patients were randomly divided into control group(n = 74)and intervention group(n = 74).The control group was given routine nursing intervention to prevent DVT during perioperative period,including routine preoperative and postoperative education,encouraging early functional exercise,using intermittent inflatable pressure pump during operation and taking preventive drugs according to doctor's orders after operation.The intervention group was given massage at Zusanli point,Taichong point and Xuehai point on the basis of the control group,and each acupoint finger was massaged twice a day for 10 min.The clinical symptoms of DVT,limb circumference,skin temperature,coagulation parameters,lower extremity blood flow and DVT incidence were measured by color Doppler ultrasound before and after operation.Excel 2016 was used to sort out the data.SPSS 25.0 software is used for statistical analysis.Results1.Analysis of risk factors for deep venous thrombosis in neurosurgery: univariate analysis results: smoking,obesity,hypertension,hemiplegia or long-term bed rest,infectious diseases,central venous catheters,endotracheal intubation or tracheotomy,mechanical ventilation,Blood transfusion,use of steroid drugs,use of dehydrating agents,use of vascular irritating drugs,prothrombin international standardized ratio,C-reactive protein,hematocrit,erythrocyte count,There was significant difference in hemoglobin and other indexes between the two groups.Multivariate Logistic regression analysis showed that infectious diseases,central venous catheters,tracheal intubation or tracheotomy,mechanical ventilation,blood transfusion,The use of steroid drugs and dehydrating agents are independent risk factors for deep venous thrombosis in neurosurgery.2.Study on the effect of postoperative nursing intervention measures for meningeal tumors: on the 3rd day after operation,the perimeter of 15 cm above patellar and 10 cm circumference of subpatellar in the intervention group were lower than those in the control group.On the first day after operation,the skin temperature of suprapatellar 15 cm in the affected limb was lower than that in the control group on the 1st and 3rd day after operation,and the number of patients with positive Homan sign and Neuhof sign in the intervention group was lower than that in the control group.The incidence of DVT in the intervention group was lower than that in the control group,and the above indexes were statistically significant(P<0.05).Conclusion1.Analysis of risk factors for deep venous thrombosis in neurosurgery:(1)Infectious diseases,central venous catheters,endotracheal intubation or tracheotomy,mechanical assisted ventilation,blood transfusion,use of steroid drugs,use of dehydrating agent,C-reactive protein,Red blood cell count may be an independent risk factor for DVT in neurosurgical patients.(2)In this study,thrombosis often occurred in unilateral lower extremities in patients with neurosurgery complicated with DVT,and the right lower extremity was the most common.The distribution of venous vessels involved in thrombus was the most common(80.4%).Meningioma(31.3%)was the most common primary disease.2.Study on the effect of nursing intervention measures after meningeal tumor operation: acupoint stimulation can improve the swelling degree of affected limbs,reduce the skin temperature of affected limbs,reduce Homan sign and Neuhof sign,and improve the coagulation index,improve lower extremity blood flow and reduce the occurrence of DVT after meningioma.
