Objective:Sinusitis is divided into acute sinusitis and chronic sinusitis.Chronic sinusitis is more common.At the same time,multiple sinuses are involved.The treatment of chronic sinusitis includes drug therapy and surgical treatment.Most patients with sinusitis have good therapeutic effect,but patients with allergic constitution,bronchial asthma,aspirin intolerance,than the disease treatment effect is poor.Nasal polyp(nasal polyp)is a soft litchi-like neoplasm on the surface of mucosa,which is located in the nasal cavity and paranasal sinus.There are often clinical manifestations such as nasal obstruction,increased secretions,facial swelling,and hypoolusia are also common symptoms.For the elderly group,the main reason is that the immune function of the elderly is generally poor,and more than 80% of the elderly patients with chronic sinusitis will have abnormal nasal anatomy at the same time,resulting in nasal secretions can not flow out,resulting in nasal cavity blocked by nasal endocrine deposits and then nasal polyps,which greatly reduces the quality of life of the elderly group.Surgery is to treat chronic sinusitis,nasal polyps.Common methods.With the continuous progress of nasal surgery technology,functional endoscopic sinus surgery has emerged.There are two kinds of endoscopic sinus surgery: functional and radical.Their effectiveness and safety are the direction of this study.Through clinical practice and comparison with surgical results,we can understand the advantages and lower risks of endoscopic sinus surgery in elderly patients with sinusitis and nasal polyps.Methods:We selected 86 patients with chronic sinusitis and nasal polyps,all of whom were elderly.The case was from a central clinical medical college of Tianjin Medical University from June 2016 to August 2018.They were divided into control group and observation group by random number method.The control group was given traditional extranasal approach to remove inflammatory lesions,retain or remove mucosa as appropriate,and then drill a proper pair of holes in the inferior nasal meatus area of the medial wall of maxillary sinus for long-term drainage.After operation,the maxillary sinus cavity still exists,and the inflammation of mucosa may heal.Even restore normal physiological function,but can not absolutely rule out the possibility of re-infection,the maxillary sinus cavity contracted by scar tissue is more likely to be re-infected in the case of poor drainage.Observation group using nasal endoscopy for functional surgical treatment,intraoperative clearance of lesions,improve sinus orificedrainage,sinus mucosa,middle turbinate as far as possible to maintain the normal physiological function of the sinus,so that Sinus heals itself.The total effective rate was compared between the two groups.Results:The experimental results showed that The effectiveness of endoscopic sinus surgery and radical surgery were significantly higher than those of endoscopic sinus surgery.,and the diversity was statistically significant(P < 0.05).According to the results of comparative statistics,Compared with the two groups,the total effective rate of endoscopic sinus surgery was 95.35%,which was significantly higher than that of radical surgery(81.40%).The difference was significant in statistics(P < 0.05).Conclusion:Functional surgery has good clinical effect.Endoscopic sinus surgery should be strengthened in elderly patients.The mucosa of nasal cavity and paranasal sinus has secretory function and mucociliary has secretory function.If we use traditional radical operation to destroy mucosa,it is more conducive to the recurrence of polyps,but the clinical effect is worse.In this regard,we have gained valuable clinical experience through practice.A series of new theories are put forward.Therefore,the treatment of senile chronic rhinitis and sinusitis deserves further promotion.
