Objective To investigate the correlation between biopsy Gleason score(GS)and radical prostatectomy(RP)Gleason score and its influencing factorsMethods We retrospectively analyzed 101 patients with prostate cancer who underwent radical prostatectomy between January 2011 to June 2018 in the General Hospital of XX Medical University.Patients who received androgen deprivation therapy,radiotherapy or chemotherapy were excluded.The biopsy Gleason score,RP Gleason score,age,body mass index(BMI),prostate specific antigen(PSA),prostate volume(PV),PSA density(PSAD),hyperlipidemia,positive Biopsy Cores were analyzed in all patients.Univariate and multivariate logistic regression analysis was used to determine influencing factors of Gleason score changes.Results were considered significant at p<0.05.Results A total of 101 patients.Compared to the Gleason score of biopsy specimens,that of prostatectomy specimens was concordant in 52 cases(51.5%),upgrading in 42 cases(41.6%),and downgrading in 7 cases(6.9%).According to univariate logistic regression analysis,prostate volume,PSAD were significantly associated with Gleason score upgrading(P < 0.05).Age,BMI,PSA,number of positive Biopsy Cores and hyperlipidemia had no significant statistical significance(P > 0.05).Multivariate Logistic regression analysis showed that prostate volume(P=0.002,OR=0.769,95%CI 0.650-0.911)was an independent predictor and inversely correlated with of RP GS upgrading.Conclusion1?There is a moderate concordance between the scores in RP and biopsy.RP GS upgrading is more common than downgrading2? prostate volume was an independent predictor and inversely correlated with of RP GS upgrading.
