Objective confirmation of the efficacy of walnut oil in the treatment of burn wounds,and exploration of the mechanism of walnut oil in the treatment of burn wounds.Methods(1)The retrospective clinical cohort study: Inpatients with burn refractory wounds were included from the Department of Burn and Plastic Surgery of our hospital between January 1,2013 and December 31,2016.The efficacy of three different treatments,walnut oil,surgery and Routine dressing on burn refractory wounds were evaluated and compared by observing the wound repair and wound healing.(2)Animal experiments: porcine models with three-degree burn wounds were established to evaluate wound healing and were treated with walnut oil,growth factors and physiological saline,respectively.Tissue morphology was observed and protein expression in wounds was detected.(3)Cell Experiments: Primary human keratinocytes and fibroblasts were isolated and cultivated.The effects of walnut oil and linoleic acid on cell proliferation of human skin keratinocytes and fibroblasts were investigated using Alamar Blue assay.Results(1)The results of the retrospective clinical cohort study showed that the walnut oil treatment and the surgical therapy significantly shorten wound healing time for burn refractory wounds(P < 0.05)and significantly improved the overall clinical efficacy rate(P < 0.05);There was no significant difference between the walnut oil group and the surgical group(P > 0.05).(2)The results of animal experiments showed that the wound healing rate was significantly higher in the walnut oil group(P < 0.05),the walnut oil treatment therapy significantly shorten the wound healing time(P < 0.05).The walnut oil treatment could restrain the scar by inhibiting the protein overexpression of P63 and CK10 in epidermis compared to the growth factor therapy(P < 0.05).(3)The results of cell experiments showed that linoleic acid significantly increased the proliferation of fibroblasts and keratinocytes(P < 0.05),wound treated with walnut oil contains various ingredients could reach physiologic healing,and walnut oil has a potential to restrain the scar.Conclusion In summary,walnut oil enhances burn wound healing and could be used as a novel burn wound healing therapy.The walnut oil promotes the proliferation of skin keratinocytes and fibroblasts to a physiologic wound healing state and reduces the scar.This study provides the key information and scientific evidence for the treatment of walnut oil on burn refractory wound,and finally lays the foundation for the development of low,efficient and new therapy with important clinical significance in promotion and application of walnut oil on burn refractory wound.
