Objective:(1)To explore the characteristics of high-risk groups of cognitive impairment and related risk factors in patients with epilepsy on cognitive impairment;(2)to explore the effects of anti-epileptic drugs monotherapy and multi-drug therapy on cognitive function,and to guide clinical drug treatment.Methods: According to the inclusion and exclusion criteria,177 adult patients with epilepsy diagnosed by neurology in the Department of Neurology of the 940 th Hospital of the People’s Liberation Army Joint Service of the People’s Liberation Army from December 2017 to October 2018 were selected.The Montreal Cognitive Assessment(MoCA)(Beijing version)was used to evaluate the cognitive function of patients.The general data of epilepsy patients,related factors of disease onset,treatment factors,mental and psychological factors were used as independent variables,and the total score of MoCA was used as the dependent variable.SPSS22.0 statistical software analyzes the correlation between the above factors and cognitive impairment in patients with epilepsy.Then,the statistically significant factors are correlated with the MoCA sub-projects to explore the characteristics of cognitive impairment.Finally,multivariate binary Logistic regression was used to analyze the correlation strength between each influencing factor and cognitive impairment.Results: 1.The participants included 177 patients with epilepsy,including 85 with cognitive impairment and 92 with no cognitive impairment.The incidence of cognitive impairment in epilepsy patients was 48.02%.2.Comparison with and without cognitive impairment,found that the educational level,age,age of onset,seizure frequency,number of drugs,and interical epileptiform discharges(IEDs)were statistically significant(P<0.05).There were no significant differences in gender,trauma history,febrile seizure history,family history,course of the disease,seizure type,and personality between the two groups(P>0.05).3.The single factor correlation analysis,the variables found that the age,the course of the disease,seizure frequency,the number of drugs,the abnormal discharge of the interical,and the psychoticism of the personality were negatively correlated with the total score of the cognitive(P<0.05),there was a significant positive correlation between the education level,the age of onset and the endogenous and extroversion in personality and cognitive total score(P<0.01).There was no correlation between neuroticism and lying in personality and cognitive total score(P>0.05).4.After multivariate logistic regression analysis,the final two factors of education level and seizure frequency entered the model,and the other variables did not enter the model.Conclusions: 1.Age,education level,age of onset,course of disease,seizure frequency,number of drugs,abnormal discharge of the interical,endogenous and extroversion in personality,and psychoticism have an impact on cognitive function in patients with epilepsy.2.The older the patient,the earlier the onset,the longer the course of the disease,the more severe the cognitive impairment when combined with the drug and the abnormal discharge during the interical;Introverted,withdrawn and stubborn patients have worse cognitive function.3.Each influencing factor has an effect on almost all sub-domains of the MoCA scale,and the cognitive function is impaired extensively.4.The frequency of seizures can independently predict the cognitive impairment of epilepsy patients.
