Objective:In this study,data mining technology was used to explore the drug types,meridities and prescription rules of the literature on the treatment of knee osteoarthritis with oral Chinese medicine in recent ten years,so as to provide reference for the clinical treatment of knee osteoarthritis with Chinese medicine.The core drug combination with the highest correlation degree and the most representative was selected,and the mechanism of its treatment of knee osteoarthritis was explored by using network pharmacology technology,providing data support and documentary basis for the experimental direction of traditional Chinese medicine treatment of knee osteoarthritis.This paper summarizes the efficacy of the core drug combination from the perspective of traditional Chinese medicine,and compares it with the possible modern medical mechanism explored by network pharmacology,so as to provide references for clinical staff to understand the efficacy of traditional Chinese medicine and develop new drugs from the perspective of modern medicine.Methods:Electronic retrieval was used to collect the literature on the clinical treatment of knee osteoarthritis with oral Chinese medicine published by Chinese journal full-text database(CNKI)in the past 10 years(January 2010--July 2019).According to the inclusion criteria and exclusion criteria,literature management software was used to select the final included literature.After extracting the information in the literature,establishing the database,normalizing the data,using Excel to input the information of the literature and prescriptions,and using statistical software to carry out frequency statistics,cluster analysis and correlation analysis on the traditional Chinese medicine.Network pharmacology was used to explore the mechanism of drug combinations with the highest correlation degree in association analysis.Results:1.After downloading 167 references,and after intensive reading and re-comparison of inclusion criteria and exclusion criteria,the remaining 127 references were included in this study,involving 149 prescriptions.2.There were 170 drugs in total,and the cumulative frequency of use was 1733 times.There are 170 traditional Chinese medicines in total,with a total usage frequency of 1733 times.The frequency of more than or equal to 20 drugs are Achyranthis Bidentatae Radix,Angelicae Sinensis Radix,licorice,Paeoniae Radix Alba,Eucommiae Cortex,Rehmanniae Radix Praeparata,Chuanxiong Rhizoma,Radix Angelicae Biseratae,Spatholobus Suberectus Dunn,Herba Taxilli and so on a total of 25 drugs.The frequency of the large categories of drugs in the top ten are tonic medicine,promoting blood circulation and removing blood stasis,dispelling wind and dampness,relieving drugs,antipyretic,clearing damp and promoting diuresis,calming liver wind,drugs for warming the interior,dampness-transforming medicinal and medicines for regulating vital energy.Frequency of drug in the classification of the small top ten are Qi-invigorating drugs,blood-activating and menstrual-regulating drugs,blood-invigorating drugs,yang-invigorating drugs,wind-cold-dampness-dispelling drugs,wind-cold-dissipating drugs,blood-activating and pain-relieving drugs,wind-dampness-dispelling and heat-removing drugs,wind-dampness-dispelling and muscles-strengthening and bones-strengthening drugs,water-promoting and edema-eliminating drugs,deficiency-invigorating drugs are mainly Qi-invigorating drugs,blood-invigorating and yang-invigorating drugs.Promoting blood circulation and removing blood stasis are mainly blood-activating and menstrual-regulating drugs,blood-activating and pain-relieving drugs.Dispelling wind and dampness drugs are mainly wind-cold-dampness-dispelling drugs,wind-dampness-dispelling and heat-removing drugs,wind-dampness-dispelling and muscles-strengthening and bones-strengthening drugs.Relieving drugs are mainly wind-cold-dissipating drugs.The total frequency of all meridian is 4228.The frequency of meridian was mainly liver,kidney and spleen.3.According to the cluster analysis,the drugs can be divided into Saposhnikoviae Radix-Asari Radix Et Rhizoma-Cinnanmomi Cortex-Cinnanmomi Cortex-Gentiana Macrophylla Pall-Radix Angelicae Biseratae-Herba Taxilli-Poria Cocos(Schw.)Wolf.-Chuanxiong Rhizoma-Eucommiae Cortex-Paeoniae Radix Alba,Cinnamomi Ramulus-Atractylodes Macrocephala Koidz-Chaenomeles Sinensis(Thouin)Koehne-Radix Clematidis-Dipsaci Radix-Drynariae Rhizoma-Epimrdii Herba-Fructus Psoraleae-Rehmanniae Radix Praeparata-Hedysarum Multijugum Maxim-Spatholobus Suberectus Dunn,Angelicae Sinensis Radix-Licorice-beef root,Achyranthis Bidentatae Radix.4.The core drug combination with the highest correlation degree was Herba Taxilli-Paeoniae Radix Alba-Angelicae Sinensis Radix-Eucommiae Cortex.5.There are 32 active components in the core drug combination,and 26 chemical components which may be effective for knee osteoarthritis include quercetin,kaeniphenol,beta-carotene and beta-sitosterol.There may be 100 effective targets such as JUN,AKT1,RELA,IL6 and MAPK1 in the treatment of knee osteoarthritis.6.The protein interaction network diagram of the core drug group in the treatment of knee osteoarthritis and the active component-target-KOA disease network diagram of the core drug group were constructed.7.There may be 1099 biological processes to treat knee osteoarthritis such as the regulation of nitric-oxide synthase biosynthetic process,response to growth factor,cellular response to interleukin-6,cellular response to transforming growth factor beta stimulus,response to phenylpropanoid and so on.There may be 251 signaling pathways,including transcriptional misregulation in cancer,fluid shear stress and atherosclerosis pathway,cytokine-cytokine receptor interaction,interaction between viral proteins and cytokines and their cytokine receptors,chemokine signaling pathway and so on.Conclusion:In the last ten years in the literature published on CNKI,the idea of treating knee osteoarthritis with traditional Chinese medicine is mainly based on the understanding of traditional Chinese medicine to treat knee osteoarthritis.The treatment is based on the three viscera of liver,kidney and spleen,with the purpose of tonifying liver and kidney,activating blood circulation and removing blood stasis,dispelling wind,cold and dampness,and strengthening muscles and bones as the treatment principle.The combination of the core drugs selected from many drugs is a collection of the ideas of traditional Chinese medicine for internal treatment of knee osteoarthritis in recent ten years.In terms of the modern mechanism,it may be caused by multiple biological processes and signaling pathways to prevent and treat knee osteoarthritis by reducing chondrocyte apoptosis,improving blood flow shear stress,anti-inflammatory,antioxidant and other mechanisms.Through the combination of data mining and network pharmacology,it can provide ideas for the selection of clinical prescription of traditional Chinese medicine for the treatment of knee osteoarthritis and the preliminary basis for the development of new drugs.
