Clinical Study On The Treatment Of Spastic Hemiplegia After Stroke With The Method Of Warming Yang,Supplementing Qi And Dredging Collaterals Combined With Tizanidine

Objective: to observe and analyze the clinical value,safety and reliability of mild Yang yiqi qi tongluo method combined with tizanidine in treating patients with spastic hemiplegia after stroke with syndrome differentiation of deficiency of Yang qi.Methods: by collecting the topic between March 2018 and January 2020 in our hospital encephalopathy ward and outpatient service 80 cases included in the standard of spastic hemiplegia sun be the spirit deficiency patients after stroke,and using random Numbers table randomly divided into observation group and control group in the two groups,both groups were given conventional stroke secondary prevention and basic western medicine symptomatic treatment,control group 40 cases: give hydrochloric acid for Tony set(starting dose of 1 mg each time,3 times a day,if no special discomfort,after 3 d doses increased to 2 mg each time,3 times per day to maintain);Based on the observation group and control group,40 cases in the control group + Wen Yang Yi gas t2 d m method(GUI huangqi guizhi wuwu: radix astragali 30 g,10 g cassia twig,and ginger 6 g,peony 30 g,big jujube,10 g earthworm 20 g,wide steel rattan 15 g,15 g suberect spatholobus stem,main licorice root 6 g,avoid the frying machine,a dose,water a blunt,morning and evening suit).8 weeks of treatment,treatment of two groups of patients before the observation indexes and baseline for comparison,observe and record before and after treatment and the curative effect of two groups of patients with sexual index and safety index,including patients with TCM syndrome integral,the U.S.National institutes of health stroke scale score(National Institute of Health stroke scale,NIHSS)Fugel hemiplegic limbs-Meyer movement function score(Fugel-Meyer Assessment FMA),Clinical spasm Index(Clinical Spasticity Index(CSI),Mo dified Ashworth cramps classification(Modified Ashworth scale MAS),daily life activities ability Modified Barthel Index(Modified Barthel Ind ex MBI),monitored vital signs,liver and kidney function,coagulation function,and electrocardiogram,observed whether there were any adverse reactions and complications,and used SPSS software for statistical analysis.Results:1.Before treatment,there was no statistical significance in gender,age,TCM syndrome score,NIHSS,FMA,CSI,MAS,MBI,disease course and risk factors in the two groups(P>0.05),indicating comparability.2.After 8 weeks of treatment,the total effective rate of clinical efficacy in the two groups was 84.62% in the observation group and 64.10% in the control group,respectively.The total effective rate of TCM symptoms was 89.74% in the observation group and 71.79% in the control group.The clinical efficacy and total effective rate of TCM syndrome in the observation group were better than those in the control group,and the difference was statistically significant(P<0.05).3.After 8 weeks of treatment,the NIHSS score,FMA score,CSI score,MAS score and MBI score of the two groups were improved to different degrees,and the observation group was better than the control group,with statistically significant differences(P<0.05).4.Safety evaluation results: before and after treatment,there were no obvious abnormalities in the general vital signs and various examination indicators of patients in the two groups;after treatment,the total incidence of adverse reactions in the observation group and the control group were 10.3% and 12.8%,respectively,with no statistically significant difference(P>0.05).Conclusion:1.The combination of warm Yang yiqi and collateral-clearing combined with tizanidine has obvious clinical efficacy in the treatment of patients with spastic hemiplegia Yang deficiency syndrome after stroke.Compared with tizanidine alone,it is more effective in improving clinical symptoms and TCM symptoms.2.The degree of improvement of nerve function defect,spasm grade,hemiplegic limb movement function,clinical spasm index and daily living activity of patients with spastic hemiplegia Yang deficiency syndrome after stroke were better than those of the control group.3.The combination of warm Yang yiqi and collateral-clearing combined with tizanidine in the treatment of patients with spastic hemiplegia syndrome of Yang deficiency after stroke showed no increase in the rate of adverse reactions,indicating that huangqi guizhi wuwu decoction plus decoction is safe in clinical use.
