Objective: Due to the particularity of the oral environment and oral mucosa structure,after oral mucosa surgery or trauma suture,the postoperative reaction is often severe.There are many factors to reduce the postoperative reaction.Among them,the choice of suture material is a more important factor.However,there are many types of sutures available on the market,which requires clinicians to make the right choice.The purpose of this study is to compare the effects of four types of suture materials on the soft tissue of oral mucosa in Beagle dogs and evaluate them.It is hoped that it can provide a certain reference for dentists to choose the appropriate suture material in future.Methods: Six 1-year-old Beagle dogs were anesthetized with intramuscular injection,3% hydrogen peroxide solution and 0.9% saline were used to wash the mouth alternately.Iodine sterilized beagle dog head and neck and surgical mucosa,local infiltration anesthesia with lidocaine.Each incision was made in the buccal mucosa of the bilateral maxillary premolar area,and a simple interrupted suture was performed sequentially in accordance with the PGA absorbable suture,nylon,silk,80 X,10%PDMS coated UHMWPE suture materials.During the experiment,strictly aseptic operation,anti-inflammatory treatment after surgery.The general situation of the oral mucosa sutures was recorded 3 days and 7 days after the operation(peripheral swelling,food adhesion,knot safety,and the wound dehiscence due to slippage of the suture).Tissues were resected in blocks.The removed samples were fixed and embedded after removing sutures.Afterwards,they were handled by HE staining.Observation of the histology of the suture around the light microscope.Statistical analysis was performed using spss20.0,and the chi-square test was used as the general observation method.When the cell value was less than 5,according to Fisher's exact probability method,a statistical difference was considered when p <0.05.Histological observations were performed using the Kruskal Wallis test.Similarly,p <0.05 was considered statistically significant.Results:1.General observation All 6 beagle dogs survived until the end of the experiment.Regardless of 3 days or7 days,in terms of knot safety,the number of the slippage of PGA absorbable sutures was the largest.Similarly,all four types of suture materials showed different degrees of inflammatory reaction,the silk showed the most intense than others.In terms of food adhesion,silk was more than the other three kinds of sutures.The food adhesion phenomenon of silk was the most obvious at 7 days.In the case of wound dehiscence,only one case of the absorbable suture showed significant wound dehiscence due to the disappearance of the suture.2.Histological observation At 3 days: the inflammatory reaction around the absorbable suture and 80 X,10%PDMS coated UHMWPE suture was mostly mild,nylon was medium,and the silk showed the phenomenon of extensive and dense infiltration of inflammatory cells around the suture,with some granulation tissue formation.At 7 days: Compared to 3days,the inflammation reaction of all sutures was aggravated,and the silk showed severe inflammatory reaction(some granulation tissue,some liquefaction and necrosis were visible),while the inflammation of nylon and 80 X,10% PDMS coated UHMWPE suture were moderately predominant,and the degree of inflammatory reaction around the absorbable suture was relatively average.In most samples,the formation of fibrous tissue was observed in 3 and 7 days without obvious epithelial tissue formation.3.Statistical analysis The general statistical results showed that in terms of knot safety at 3 days and 7days,the difference between using absorbable suture,nylon,silk,80 X,10% PDMS coated UHMWPE suture was statistically significant(P = 0.026),and the absorbable suture is less secure.There were no statistically significant differences in peripheral swelling of four sutures,the P values at 3 and 7 days were P=0.793 and P =0.141,respectively.When wound dehiscence was considered,there was no statistical difference in the effect of the four sutures between 3 and 7 days(P = 1.000).At 3 days,there was no statistically significant difference in food adhesion between the other three sutures and absorbable sutures(P = 0.314).At 7 days,the four sutures had statistical differences(P = 0.011).The food adhesion of silk was significantly higher than the other three kinds of sutures.Histological statistics were tested by the Kruskal Wallis method,and the results showed that: 3,7 days of silk was significantly higher than the other three sutures(p<0.05);there was no significant statistical difference between the other three sutures(p> 0.05).There was no statistically significant difference between epithelial formation,fibrous tissue formation,and granulation tissue formation of the four sutures at 3 and 7days(p> 0.05).Conclusion:1.The food adhesion of silk is the most serious.Silk can cause more serious tissue reaction,and it is easy to form suture abscess.2.The knots of the absorbable suture have low safety.3.There is no difference in the inflammatory response of nylon,absorbable suture,80 X,10% PDMS coated UHMWPE suture,all of which show mild to moderate inflammatory reaction.
