| Objective:By comparing the surgical resection rate,operation time,postoperative hospital stay,cerebrospinal fluid leakage,diabetes insipidus,infection and other related postoperative complications in patients undergoing neuro-endoscopic and microscopic transsphenoidal pituitary adenoma resection A series of indicators to evaluate the difference between neuro-endoscopic transsphenoidal pituitary adenoma resection and microscopic transsphenoidal pituitary adenoma resection,provide a certain reference value for clinical surgical treatment options.Methods:A retrospective collection of 146 cases of pituitary tumor patients admitted to the Department of Neurosurgery,Second Affiliated Hospital of Kunming Medical University from January 2016 to October 2018.According to different ways of surgery,there were 62 cases in the endoscopy group and 84 cases in the microscope group.By comparing and analyzing the age,sex,tumor resection,operation time,postoperative hospital stay and postoperative complications(diabetes insipidus,cerebrospinal fluid leakage,infection)in the two groups,SPSS software was used for analysis and measurement data.Mean ± standard deviation(X±S)using t test;count data expressed in rate(%),using x 2 test,P<0.05 was statistically significant.Results:The total rate of neuroendoscopy was higher than that of the microscope group(87.10%vs 83.33%),χ2=0.395,P=0.530,and there was no significant difference.The neurological endoscopy group was longer than the microscope group(101±18 VS 81±11),t=8.157,P=0<0.05,there was significant difference.The postoperative hospitalization time was lower in the neuroendoscopy group than in the microscope group(4.31±0.72 days vs 6.08±1.27 days),t=2,397,p=0.023,significant Differences.Postoperative complications:the cerebrospinal fluid leakage rate was higher in the neuroendoscopy group than in the microscope group(6.45%vs 5.95%),X2=0.015,P=0.901,no significant difference;the incidence of diabetes insipidus neuroendoscopy group was lower than the microscope group(4.84%vs 7.14%),X2=0.327,P=0.567,no significant difference;the incidence of infection was higher in the neuroendoscopy group than in the microscope group(3.23%vs 1.19%),X2=0.734,P=0.392,no significant difference.Conclusion:Under the contrast microscope,the transsphenoidal pituitary adenoma resection,the endoscopic surgery of the transsphenoidal pituitary adenoma was significantly longer,but the postoperative hospital stay was significantly shorter than the former.There were no significant differences between the two groups in terms of general condition,total tumor resection rate,postoperative complications,and improvement in postoperative hormone levels.In general,neuroendoscopic surgery is still in the process of development.With the continuous improvement of endoscopic techniques,the characteristics of small damage and rapid recovery will become more and more obvious,and it is more in line with the development trend of local neurosurgery.It will also become more and more extensive. |
