ObjectiveDeep venous thromboembolism(DVT)is the common complication flowing keen or hip replacement,it's fatal if it can't be cured and develop into the pulmonary embolism(PE).Although the incidence rates of DVT has been reduced by using anticoagulant drug unite the mechanical anticoagulant therapy.There are some patient can be checked out the DVT by color ultrasound or the clinical symptoms.The study is to establish a better anticoagulation program by exploring the safety and efficacy of aspirin unite low-molecular-weight heparin on venous thromboembolism flowing knee or hip replacement.MethodA total of 200 patients under the one-sided knee replacement and 140 patients under the one-sided hip replacement from August 2015 to September 2017 were recruited into this prospective study.Half of them use the Bayaspirin unite low-molecular-weight heparin calium to prevent DVT(experimental group),the other only use the low-molecular-weight heparin calium to prevent DVT(control group).We compared the thrombus rate,platelet,hemoglobin,prothrombin time(PT),activated partial thromboplastin time(APTT),DDimer,antithrombin and fibrinogen degradation product(FDP)of the two groups.ResultThe rate of the DVT of the patients following the one-sided knee replacement of the experimental group is 43%,the rate of the group of the patients following the one-sided hip replacement is 34%.The rate of the DVT of the patients following the one-sided knee replacement of the control group is 50%,the rate of the group of the patients following the one-sided hip replacement is 37%.The rate of DVT of the experimental group is lower than the rate of the control group under the same operation,but there is no statistically significant(P>0.05).We check the blood routine examination and mechanism of coagulation 7 days after the operation.The PT and FDP of the experimental group is higher than the control group.The hemoglobin of experimental group is lower than the control group.Difference was statistically significant(p<0.05).The difference of other indicators of the blood was not statistically significant.Conclusion1.The rate of the DVT of the patients with the one-sided knee replacement is higher than the patients with the one-sided hip replacement.2.The incidence rates of DVT has been reduced by using anticoagulant drug unite the mechanical anticoagulant therapy,but there is still some patients with the asymptomatic myothrombosis.3.Aspirin unite the low-molecular-heparin has no clear advantage in the prevention of the DVT flowing the one-sided knee or hip replacement against single anticoagulant of low-molecular-heparin.And the combination of the low-molecular-heparin and asprin can decrease the hemoglobion and prolong coagulation time.
