Objective:To observe the Traditional Chinese Medicine(TCM)syndrome clinical scores,TCSS scores,nerve conduction velocity,hemorheology,blood glucose and clinical effect in the treatment of diabetic peripheral neuropathy(DPN)with Mudan Granules combined with Mecobalamin and Epalrestat,so as to provide an effective treatment method for the prevention and treatment of diabetic peripheral neuropathy by combining traditional Chinese and western medicine.Methods:Sixty cases of DPN patients were selected as the clinical study objects and randomly divided into western medicine control group and integrated traditional and western medicine treatment group,with 30 cases in each group.The two groups were given basic treatment for diabetes(diabetes education,diet and exercise therapy and oral hypoglycemic drugs or insulin),the control group was treated with Mecobalamin(Qixin)capsule combined with Epalrestat(Tanglin)tablet,and the treatment group was treated with Mudan granule combined with Mecobalamine(Qixin)capsule and Epalrestat(Tanglin)tablet.The TCM syndrome score,nerve conduction velocity,hemorheology,blood glucose changes and clinical effect of the two groups of patients were observed.Statistical analysis was performed to evaluate the efficacy of Mudan granule combined with Mecobalamin and Epalrestat in the treatment of diabetic peripheral neuropathy.Results:1.Comparison of TCM syndrome clinical scores:In terms of decreasing TCM syndrome clinical scores,the degree of decreasing TCM syndrome clinical scores in the treatment group was significantly better than that in the control group(P<0.01).In improving clinical symptoms of TCM,the treatment group patients with physical pain,numbness,sweat,languid,shortness of breath lazy speech,lumbar debility symptoms were significantly improved than before treatment(P<0.05),while the control group after treatment only numbness and pain improved(P<0.05).Other symptoms,such as sweat,languid,shortness of breath lazy speech and lumbar debility,were not seen obviously improved(P>0.05).At the same time,the treatment group was significantly better than the control group in improving limb pain,numbness,sweat,languid,shortness of breath lazy speech and lumbar debility symptoms(P<0.05).2.Comparison of Toronto clinical scoring system:After treatment,scores of Toronto clinical scoring system were significantly lower in both groups(P<0.01),and scores in the treatment group were lower than those in the control group(P<0.05).3.Comparison of nerve conduction velocity:After treatment,the median nerve,common peroneal nerve MNCV and SNCV conduction velocity of the two groups of patients were significantly improved(P<0.01),and the treatment group was significantly better than the control group(P<0.01).4.Comparison of hemorheology:After treatment,there were significant differences in the viscosity of high whole blood,low whole blood and plasma between the two groups(P<0.01)After treatment,there were significant differences in the indexes of hemorheology between the treatment group and the control group(P<0.05).5.Comparison of blood glucose changes:There was no significant difference in FBG,2hPG and HbA1c between the two groups after treatment and before treatment(P>0.05),and no significant difference in FBG,2hPG and HbA1c between the treatment group and the control group compared with that before treatment(P>0.05).6.Comparison of clinical efficacy:Comparison of curative ettect of TCM syndromes:In the control group,there were 0 cases of clinical recovery,1 case of obvious effect,8 cases of effective,18 cases of ineffective,the total effective rate was 33.33%.In the treatment group,there were 7 cases of clinical recovery,9 cases of obvious effect,10 cases of effective,3 cases of ineffective,the total effective rate was 89.65%,the difference was statistically significant(P<0.01).Comparison of comprehensive efficacy:In the control group,there were 5 cases of obvious effect 14 cases of effective,8 cases of ineffective,the total effective rate was 70.37%.In the treatment group,there were 8 cases with obvious effect,19 cases with effective effect and 2 cases with ineffective effect,the total effective rate was 93.10%,and the difference was statistically significant(P<0.05).Comparison of long-term efficacy:6 cases of recurrence in the control group,with a recurrence rate of 22.2%;In the treatment group,there was 1 case of recurrence,with a recurrence rate of 3.4%.The difference was statistically significant(P<0.05).Conclusion:Mudan Granules combined with Mecobalamin and Epalrestat can significantly improve the clinical symptoms in patients with diabetic peripheral neuropathy,lower TCM clinical score,improve the nerve conduction velocity,increase blood fluxion,to treat diabetes peripheral neuropathy has good clinical curative effect,safe and effective,without obvious adverse reactions.
