Objective :To investigate the clinical effect of the lower extremity Dellon triple nerve decompression in the treatment of early diabetic Charcot foot.Methods :24patients with diabetes Charcot foot in Eichenholtz stage 0 to 1 treated in our hospital from September 2018 to September 2019 were selected,including 20 males and 2 females,20 patients in Eichenholtz stage 0 and 4 patients in stage 1.5 cases of Brodsky type I,11 cases of type II,3 cases of type III,2 cases of type IV,3 cases of type V.14 patients received Lower extremity Dellon triple nerve decompression treatment was divided into the experimental group,and 10 patients were treated with conservative treatment of limb immobilization,nutritional nerve drugs and so on.Collect baseline data of all patients,compare the two groups of patients with dual energy X-ray absorptiometry(DXA),lower limb electromyography(EMG),and two-point discrimination(TPD)results before and6 months after treatment,and bone quality Looseness,differences in nerve conduction speed,and skin sensation were compared and analyzed.Results: All patients were followed up for 6 to 12 months,with an average of 6.5 months.There were no statistically significant differences in the age,duration of diabetes,and preoperative blood glucose levels between the two groups(P> 0.05),and they were comparable.All the incisions in the experimental group were healed in primary stage.The comparison of nerve conduction velocity between the common peroneal nerve,tibial nerve and deep peroneal nerve in the two groups at 6 months after operation showed that the improvement of the nerve conduction velocity in the experimental group was significantly better than that in the control group,and the difference between the two groups was statistically significant(P<0.05).).Compared with before treatment,the improvement of the two-point discrimination of the sole skin of the experimental group was significantly better than that before the treatment at 6 months after treatment,and the difference was statisticallysignificant(P <0.05),while the difference in the control group was not statistically significant..There was no significant difference in bone mineral density(T-score)before treatment between the two groups of patients(P> 0.05).The improvement of bone mineral density(T-score)in the experimental group at 6 months after treatment was better than that in the control group,and the difference between the two groups was statistically significant(P<0.05).Conclusion: The lower extremity Dellon triple nerve decompression can improve the Charcot foot symptoms of Eichenholtz stage 0 ~ 1 diabetes mellitus,which has the advantages of less trauma,quick recovery and fewer complications,and has certain clinical application value.
