Objective:1.To study the correlation between LPA,COX-2,CXCR3,HIF-1 ?,Pt,TT,hs CRP and syndrome types in IPF patients with phlegm heat syndrome and Qi Yin deficiency and stasis syndrome.2.To study the effect of Shenqi Cordyceps Sinensis Recipe on the lung pathology and the expression of VEGF,PI3 K,Akt and HIF-1 ? m RNA in IPF rats.Based on the mechanism of regulating angiogenesis factor in IPF rats,the effect of Shenqi Cordyceps Sinensis Recipe on pulmonary fibrosis was studied.Methods:1.Clinical research: collect 30 cases of IPF with phlegm heat damming lung syndrome and Qi Yin deficiency syndrome and stasis syndrome respectively,The control group was 30 normal checkups,collect the serum LPA,COX-2,CXCR3,HIF-1 ?,Pt,TT,hs CRP of the three groups to compare the correlation between syndrome types.2.Animal experiment: 128 rats were randomly divided into losartan group(L),Shenqi Cordyceps Sinensis Recipe group(S),blank group(K),and model group(M),with 32 in each group.Rats in the blank group were treated with sham operation by intratracheal instillation of physiological saline of equal volume.Rats in the other three groups were treated with the same method by intratracheal instillation of bleomycin for modeling.Materials were differentiated on the 7th,14 th,21st and 28 th days after modeling The expression of VEGF,PI3 K,Akt and HIF-1 ? were detected by RT-PCR.Result:1.Clinical research1.1 The serum Pt content of patients with IPF was lower than normal group,and the serum Pt content of patients with deficiency of Qi and Yin combined with stasis group was significantly higher than that of patients with phlegm heat stagnation in Lung Group and normal group,normal group,the serum hs CRP of patients with deficiency of Qi and Yin combined with stasis group and phlegm heat stagnation in lung group was significantly higher than normal group,with statistical difference,and the serum hs CRP of patients with IPF was also different between the two groups,including phlegm heat stagnation Compared with the normal group,the serum TT content of IPF patients in the two groups was lower,while that in the phlegm heat damming Lung Group was slightly higher than that in the deficiency of Qi and Yin and stagnation of stasis group,with no significant difference.1.2 LPA,COX-2,CXCR3 and HIF-1? in the serum of IPF patients in the two groups were significantly higher than normal group,especially the difference between CXCR3 and HIF-1 ? in the two groups;compared with the phlegm heat dammed Lung Group,LPA and COX-2 in the serum of the patients in the deficiency of both qi and Yin and the stagnation of blood stasis group were higher in different degrees,LPA and COX-2 in the serum of the phlegm heat dammed lung group were higher than those in the deficiency of both qi and Yin The difference was statistically significant.2.Animal experiment2.1 General situation: after the establishment of the model,the state of the blank group was the best at the same time point,and the state of the model group was worse with the increase of time.The state of the Losartan group at the same time point had no significant difference with that of the Shenqi Cordyceps Sinensis Recipe,but it was superior to the model group.2.2 HE staining: on 7th,14 th,21st and 28 th day after modeling,the alveolar tissue structure of the blank group was basically normal,clear alveolus cavity,without inflammatory cell infiltration,without edema,and no obvious thickening of alveolar septum.In the model group,on the 7th day after the establishment of themodel,there were edema and widening of the alveolar septum,and inflammatory cell infiltration in the alveolar cavity and the bronchial wall,including neutrophils,lymphocytes and macrophages.On the 14 th day,the inflammatory exudation and widening of the alveolar septum were more severe than before,and fibroblast proliferation and collagen deposition could be seen in the interstitial lung.On the 21 st day,the infiltration of inflammatory cells in alveoli was less than that on the 14 th day.At 28 days,the structure of the alveoli was disordered,even the alveoli collapsed,fibroushyperplasia and collagen deposition became more and more serious,and inflammatory cells were rare.At the same time point,the pathological changes of Shenqi Cordyceps Sinensis Recipe group and losartan group were similar to those of model group,but at the same time point,the pathological changes of Shenqi Cordyceps Sinensis Recipe group were lighter than that of losartan group.2.3 Masson staining: in the 7th,14 th,21st and 28 th sky white group,Lung tissue structure is basically normal,and no obvious blue staining area was found in the alveolar cavity.In the model group,the area of the blue collagen fibers around the blood vessel wall and trachea wall gradually increased and the color became darker.Especially on the 28 th day,large areas of dark blue collagen fibers were observed.At the same time,compared with the model group,Shenqi Cordyceps Sinensis Recipe group and losartan group area of blue collagen fibers around the lung interstitium was smaller and the color was lighter.Among them,the degree of Shenqi Cordyceps Sinensis Recipe blue staining area is lighter.2.4 The results of RT-PCR prompted that the expression of VEGF,PI3 K,Akt and HIF-1 ? in the lung tissue of the blank group,the Shenqi Cordyceps Sinensis Recipe grou,the Losartan group and the model group increased in different degrees,model group is particularly prominent(P<0.05).The expression levels of VEGF,PI3 K,Akt and HIF-1 ? in lung tissues of the blank group were similar at different time periods(P>0.05).VEGF,PI3 K,Akt and HIF-1 ? were higher in the model at the same time,(P<0.05).However,the expression of HIF-1 ?,PI3 K,Akt and VEGF in lung tissue was not significantly different between the Shenqi Cordyceps Sinensis Recipe group and the Losartan group at the same time point(P>0.05).Conclusion:1.IPF belongs to "lung Bi" and "lung flaccid" in traditional Chinese medicine,with stasis blocking lung collaterals throughout the disease.There was blood agglutination in IPF patients,and the blood hypercoagulability in the group of deficiency of Qi and Yin and stasis between the two syndromes was more significant than that in the group of phlegm heat damming lung syndrome.2.The inflammatory factors,angiogenesis promoting factors and inhibiting factors in the body may be closely related to the pathogenesis of IPF,which belongs to the syndrome of phlegm heat damming lung,deficiency of both qi and Yin and stasis.3.Shenqi Cordyceps Sinensis Recipe may inhibit the high expression of angiogenic factor and regulate the mechanism of IPF angiogenesis effectively.4.Shenqi Cordyceps Sinensis Recipe may improve the inflammation and collagen deposition of rat lung tissue,and delay lung fibrosis.
