Background:Breast cancer has become the number one issue affecting women's health.In recent years,the country has launched a national screening program for two cancers among women.The goal is to find early breast cancer patients.Early detection and early treatment is the preferred method to reduce breast cancer mortality.At present,there are various imaging examination methods,but mammography or X-ray examination has become the primary auxiliary examination method.The accuracy of the examination method is of great significance for disease screening or clinical diagnosis,and it has important clinical guiding significance for preoperative evaluation,surgical method selection and prognostic evaluation.Objective:This study analyzed the coincidence rate(ie,accuracy rate or detection rate)of breast color Doppler ultrasound and X-ray diagnosis in early breast cancer.The detection rate of color Doppler ultrasound and X-ray examination in different pathological tissue types and molecular typing.To explore the relationship between color Doppler ultrasound and X-ray imaging signs and pathological classification.The goal is to improve its diagnostic rate for early breast cancer.Method:This is a retrospective analysis.A total of 360 patients with early breast cancer who were admitted to the Department of Breast Surgery of the Third Affiliated Hospital of Zhengzhou University from January 01,2014 to December 31,2018.Patients ranged in age from 26 to 82 years,with an average age of(50 ± 11)years.Pathological results were used as the gold standard,and mammography and X-ray examinations must be performed before surgery.Calculate and compare the detection rates of breast color Doppler ultrasound and X-rays alone or in combination in different pathological tissue types and molecular types.The relationship between imaging signs in different pathological tissue types and molecular typing was analyzed.Result:1.Detection rate of different pathological tissue typesColor Doppler ultrasound accuracy rate:Non-special invasive carcinoma(88.6%)>Microinvasive breast cancer(78.6%)>Special invasive carcinoma(75.0%)>Ductal carcinoma in situ(67.6%).The detection rate of non-special invasive cancer was significantly different from the detection rate of ductal carcinoma in situ and microinvasive breast cancer(P<0.05),but there was no statistical difference between the other two groups(P>0.05).X-ray accuracy:Non-special invasive cancer(81.1%)>Microinvasive breast cancer(79.6%)>Ductal carcinoma in situ(70.8%)>Special invasive cancer(58.3%).The accuracy of X-ray examination was not statistically significant between the two groups in the four pathological tissue types(P>0.05).The accuracy rate of breast color Doppler ultrasound combined with X-ray examination:Non-special invasive cancer(96.2%)>Microinvasive breast cancer(91.8%)>Ductal carcinoma in situ(90.8%)>Special invasive cancer(83.3%).There was no statistical difference between the two groups(P>0.05).2.Detection rate of different molecular typesColor Doppler ultrasound accuracy rate:Basal-like(91.3%)>Luminal A(84.4%)>Luminal B(HER-2 negative)(75.5%)>HER-2 positive(HR negative)(74.4%)>HER-2 positive(HR positive)(73.7%).The diagnostic accuracy of Basal-like breast cancer patients was statistically different from that of Luminal B(HER-2 negative),HER-2 positive(HR negative),and HER-2 positive(HR positive)(P<0.05).X-ray accuracy:HER-2 positive(HR negative)(86.0%)>Luminal B type(HER-2 negative)(79.2%)>Luminal A type(77.2%)>Basal-like type(76.1%)>HER-2 was positive(HR positive)(73.7%),but the accuracy of X-ray examination was not statistically different between the two groups in the five molecular types(P>0.05).Ultrasound combined with X-ray examination:Basal-like(97.8%)>HER-2 positive(HR negative)(95.3%)>Luminal A(95%)>HER-2 positive(HR positive)(89.5%)>Luminal B(HER-2 negative)(86.8%).There was no statistical difference between the two groups(P>0.05).3.Comparison of mammography and combined X-ray examination with individual examinationAmong 360 patients with early breast cancer,294 were positive for color Doppler ultrasound with a detection rate of 81.7%;281 were positive for mammography with a detection rate of 78.1%;331 cases were detected with color Doppler ultrasound combined with X-ray and the detection rate was 93.5%.The color Doppler ultrasound combined with X-ray examination was statistically significant compared with color Doppler ultrasound or X-ray alone(P<0.05),while the color Doppler ultrasound had no significant difference with X-ray examination(P>0.05).In carcinoma in situ and early invasive carcinoma,the accuracy of mammography is higher than that of color Doppler ultrasound,but there is no statistical difference between the two methods(P>0.05);while in non-special invasive carcinoma and special In invasive carcinoma,the accuracy of color Doppler ultrasound is higher than that of X-ray examination,and the comparison of the two imaging methods has a statistically significant difference in non-special invasive carcinoma(P<0.05).The accuracy of color Doppler ultrasound combined with X-ray examination in different pathological tissue types is higher than that of color Doppler ultrasound or X-ray alone.Among them,in situ cancer,early invasive cancer,and non-special invasive cancer,the combined examination was statistically different from the individual examination(P<0.05).In Luminal A and Basal-like breast cancer patients,the accuracy rate of color Doppler ultrasound is higher than that of X-ray photography,and the comparison of the two imaging tests has a statistically significant difference in Basal-like patients(P<0.05);but In Luminal B type(HER-2 negative)and HER-2 positive(HR negative),the accuracy of mammography was higher than that of color Doppler ultrasound,and there was no significant difference between them(P>0.05).The accuracy of the two combined inspections in different molecular typing is higher than that of the individual inspections.Among them,the color Doppler combined X-ray examination has a statistically significant difference in Luminal A type(P<0.05),and the color Doppler combined X-ray examination has a statistical difference compared with the color Doppler in HER-2 positive(HR negative)P<0.05),there is a statistically significant difference between the combination of the two and X-ray examination in the Basal-like type(P<0.05).Couclusion:1.The diagnostic rate of color Doppler ultrasound combined with molybdenum target detection for early breast cancer with different pathological tissue types and molecular types is higher than that of individual tests.2.For non-special invasive cancer,the detection rates of color Doppler ultrasound and X-ray are higher than other pathological tissue types,and the diagnostic value of color Doppler ultrasound is higher.
