Objective:The topic survey the clinical diagnoses and pathological types,and their correlation to TCM syndromes in the IgA Nephropathy patients with renal biopsy in Chinese medicine hospital in hubei province,to explore the correlation between clinical indicators and the microscopic pathological nature of the kidney and provides a basis for objectifying the TCM syndromes of kidney diseases and adopting the combination of traditional Chinese and western medicine.Method:63 cases of primary IgAN aggregately definited by renal biopsy in Chinese medicine hospital in hubei province from January 2012 to December 2019 were collected.The patients' complete history of TCM syndromes,clinical and pathological data were retrospectively studied,and The statistics were analyzed by SPSS26.0 software to compare t heir clinical,The pathological characteristics and the correlation of TCM syndrome type.Results:1.IgA nephropathy accounted for the highest proportion(43.2%) among the primary kidney diseases.Patients with spleen-kidney Yang deficiency syndrome and liver-kidney Yin deficiency syndrome are older and have a longer course of disease.2.The severity of IgAN urinary protein was associated with hypertension,serum uric acid,albumin,glomerular segmental sclerosis,and crescent body.3.Among TCM syndromes,IgA nephropathy was characterized by qi and Yin deficiency,as well as damp-heat and blood stasis.Patients of the spleen and kidney Yang deficiency syndrome,liver and kidney Yin deficiency syndrome and damp-turbidity syndrome showed severer pathological changes,by Lee classification,most of them belonged to grade ?~?(80.0%,83.3%),the highest total scores in Katafuchi scores,and more serious in renal tubular atrophy and interstitial fibrosis and lower level of e GFR and ALB and higher level of UTP than other syndromes.4.The degrees of fusion of podocytic process was negatively correlated with e GFR,and IgA nephropathy with extensive fusion group of podocytic process was mainly characterized by spleen and kidney qi deficiency as well as damp-heat and blood stasis.Conclusion:1.The most common kidney disease with primary glomerular disease is IgAN,and the older the patient was,the longer the course was,and the more severe the symptoms was.2.Hypertension,serum uric acid,albumin,glomerular segmental sclerosis,and crescent body were risk factors for IgAN urinary protein levels.3.In IgA nephropathy,the degree of qi deficiency in human body was related to the degree of renal pathological changes,and the degree of renal pathological changes was more serious in patients with damp-turbidity syndrome.4.the extensive fusion of podocytic process might be an influential factor for the poor prognosis of IgA nephropathy.
