Background Ovarian cancer is one of the most common malignant tumors in the female reproductive system.Among them, epithelial ovarian cancer(EOC),which first appears in the ovary,is most common. In the later stages, even with a systematic and standardized treatment plan, its 5-year survival rate is low,and the mortality rate ranks first among the three major malignant tumors of the female reproductive system.Studies have shown that early ovarian epithelial cancer(stage I-II)has a high survival rate.If early detection of ovarian cancer can significantly improve the survival rate of patients.Serum tumor markers have always been a hot spot in ovarian cancer research.Carbohydrate antigen 125(CA125)is derived from epithelial tissue, which has been widely used in the diagnosis, treatment, and prognosis of epithelial ovarian cancer, but studies have shown that serum CA125 is endometriosis, digestive system disease, mesothelial tissue disease, autoimmunity Disease and early pregnancy will increase, and the specificity of single test is poor.Relevant literature indicates that Human epididymis protein 4(HE4)as a new tumor marker has obvious advantages in the diagnosis of ovarian epithelial cancer, and related scholars have found that the combined detection of the two is conducive to improving the diagnosis of ovarian epithelial cancer.Specificity and sensitivity, but the impact of clinically relevant factors on both is not clearly stated.Purpose This study explores the correlation between combined and single test serum HE4 and CA125,and compares the diagnostic efficacy evaluation to provide clinical guidance for the early diagnosis and treatment of ovarian epithelial cancer and prognostic evaluation.Method A total of 113 patients who underwent surgical treatment for pelvic masses from October 2017 to August 2019 in the First Affiliated Hospital of Xinxiang Medical College were selected according to the postoperative pathological results: 53 cases of epithelial ovarian cancer group and benign epithelial ovarian tumors 60 cases in group.Blood was drawn before the operation to detect serum HE4 and CA125 by electrochemiluminescence, and to explore the clinical relevance of the combined detection and separate detection of epithelial ovarian cancer.Result 1.The levels of serum HE4 and CA125 in epithelial ovarian cancer group were significantly higher than those in benign ovarian tumor group(P<0.05).2.Serum CA125 level in patients with epithelial ovarian cancer is positively correlated with serum HE4 concentration(r =0.013,P<0.05);3.The specificity and positive predictive value of single test serum HE4 for diagnosis of epithelial ovarian cancer are higher than CA125(P<0.05).The diagnostic specificity, positive predictive value and negative predictive value of the two combined tests are higher than single test(P<0.05).4.In patients with epithelial ovarian cancer, the level of serum HE4 was not related to age(P>0.05);the levels of serum HE4 and CA125 in patients with poorly differentiated ovarian epithelial cancer were higher than those of moderate and well differentiated(P<0.05),intermediate differentiation is not statistically significant compared with high differentiation(P>0.05);the level of both in advanced epithelial ovarian cancer(stage III,IV)is higher than that in early stage ovarian epithelial cancer(stage I,II)(P<0.05);the level of both in patients with serous ovarian cancer was higher than that in non-serous cancer(P<0.05).Conclusions 1.In epithelial ovarian cancer, the detection of serum HE4 combined with serum CA125 is better than the detection of single serum HE4 or serum CA125.The combined detection of both can improve the diagnosis rate of epithelial ovarian cancer;2.The level of serum HE4 is related to the surgical pathological stage and the degree of lesion differentiation: the later the surgical pathological stage, the lower the degree of lesion differentiation, the higher the serum HE4 level, the worse the prognosis.
