| ObjectiveThis study through the determination of HE4,TK1 and CA125 in patients with endometriosis cyst,ovarian cancer and the expression of peripheral blood,evaluation of serum HE4,TK1,CA125 and HE4 and CA125 both joint in the diagnosis of endometriosis cyst and ovarian cancer.methodsRandomly selected from January 2014 to March 2015 from anhui medical university first affiliated hospital of department of gynaecology treated and the clinical pathological diagnosis of 71 patients with endometriosis cyst for endometriosis cyst group;Who handles the postoperative pathological diagnosis of ovarian cancer patients,65 cases of ovarian cancer groups.Randomly selected from the same period in the first affiliated hospital of anhui medical university health physical examination of 70 cases of healthy women as healthy controls.Age is no statistical difference between three groups.Were determined endometriosis cyst and ovarian cancer group,healthy control group of serum HE4,TK1 preoperative and CA125 level,three groups were compared between the three tumor index for statistically.Draw serum HE4,TK1 and the ROC curve of ovarian cancer CA125 detection,more tumor target area under the ROC curve(AUC),will be about an index,sensitivity + 1)at the time of maximum serum HE4 and CA125 and TK1 level as the best threshold of diagnosis of ovarian cancer,comparing both sensitivity and specific degrees at this time.results1.HE4,TK1 level in different syndrome group were slightly higher than the control group,but comparing the two groups(P > 0.05;Ovarian cancer group serum HE4,TK1 level and its positive rate is slightly higher than the control group,compared with the control group(P < 0.05);Serum CA125 level and its positive rate in different disease group were higher than that of control group,compared with the control group(P < 0.05),serum CA125 level and its positive rate of ovarian cancer group were higher than the control group(P < 0.05).HE4 in combination with CA125 in the diagnosis of endometriosis cyst of sensitivity and specific degrees were significantly higher than that of HE4 and CA125 stand-alone diagnosis(P < 0.05),it is concluded that joint HE4 serum CA125 detection is pelvic endometriosis(EMT)and pelvic malignant lesions in the differential diagnosis of serological indicator of good.2.Serum HE4 and CA125 and TK1 level and diagnosis of the disease patients,specific degree of sensitivity,the coincidence rate is respectively: 72.31%(47/65),71.83%(51/71),72.06%(98/136);73.85%(48/65),76.06%(54/71),75.00%(102/136);70.77%(46/65),63.38%(45/71),66.91%(91/136).Serum HE4 in combination with serum CA125 sufferer at different diagnostic sensitivity,specific,coincidence rate was 90.77%(59/65),88.73%(63/71),89.71%(122/136).Are higher than that of HE4 and CA125 diagnosis separately,and the difference was statistically significant(P < 0.05).Serum HE4 and CA125,TK1 level in patients with ovarian cancer diagnosis sensitivity,specific degrees,coincidence rate was 89.23%(58/65),87.32%(62/71),88.24%(120/136);69.23%(45/65),74.65%(53/71),72.06%(98/136);76.92%(50/65),66.20%(47/71),71.32%(97/136).The diagnostic sensitivity,specificity and coincidence rate of serum HE4 combined with serum CA125 in patients with ovarian cancer were 92.31%,90.14%(64/71),and 91.18%(124/136),respectively.All were higher than HE4 and CA125,and the difference was statistically significant(P < 0.05).3.The optimal threshold value for ovarian cancer was the serum HE4,CA125,and TK1 levels in patients with the highest sensitivity(sensitivity + specificity-1).Serum HE4,CA125,TK1 in the diagnosis of ovarian cancer,when the diagnostic value of HE4 was 68.35 pmol/L,the diagnostic sensitivity was about 89.23%,and the specificity was 87.32%;When the CA125 diagnosis value of 264.54U/ml,the diagnostic sensitivity was 69.23%,specificity was 74.65%;when the TK1 diagnosis value of 0.95pmol/L,the diagnostic sensitivity was 76.92%,specificity was 66.20%;the diagnostic value of HE4 was better than CA125,TK1.Conclusion 1.Serum HE4 combined with CA125 can significantly improve the diagnostic rate of pelvic endometriosis(EMT).2.Serum TK1 has no significant clinical significance in the diagnosis of endometriosis.3.Serum HE4 is valuable in diagnosis of ovarian cancer. |
