ObjectiveTo explore the impact of family-centered empowerment discharge plan on the physical rehabilitation,self-efficacy,family function and quality of life of patients with preventive colostomy,and to find a suitable discharge plan for patients with preventive colostomy Provide reference for continued patient care.MethodsFrom December 1,2018 to March 1,2019,70 cases of preventive enterostomy patients in gastrointestinal surgery of a top three hospital in Shaoyang City were selected by the method of convenient sampling test research,randomly divided into control group and observation the group,due to the loss of sample size,the final intervention group was selected(33 cases)and the control group(32 cases).The control group patients with preventive enterostomy implemented a routine discharge plan,and the intervention group patients with preventive enterostomy implemented a discharge planintervention based on the family-centered empowerment model discharge plan.Observation of the complications of enterostomy postoperative and intestinal complications at discharge was used to observe the changes in the intervention group and the control group;self-efficacy scale ? family function scale and Chinese version of the ostomy patient quality of life scale evaluated the self-efficacy,family function and quality of life of prophylactic enterostomy patients before intervention,at discharge,4 weekends after discharge,and 12 weekends after discharge.SPSS18.0 data was used for input and analysis.The analysis methods included descriptive analysis,t test,chi-square test,rank sum test,and repeated measures analysis of variance.Results(1)Before the intervention,the general demographic data,physiological indicators,self-efficacy scale of the ostomy patient,family function scale and Chinese version of the ostomy patient quality of life scale were compared between the two groups of patients.(2)After intervention,the incidence of ostomy complications and intestinal complications during hospitalization between the two groups of patients was statistically significant(P <0.05).(3)At 4 weekends after discharge,the total score of the self-efficacy scale of the ostomy patients in the intervention group(82.03 ± 27.95)was higher than that of the control group(70.59 ± 19.08);the total score of the self-efficacy scale of the ostomy patients in the intervention group(91.00 ±25.41)higher than the control group(73.97 ± 19.26);T test results showed that at the time of discharge,4 weekends after the intervention,and 12 weekends after the intervention,the intervention group had “ total self-efficacy scores”,“social self-efficacy”,and “dietary selection efficacy”."," Sex life satisfaction performance "," keep vitality performance "," ostomy self-care efficiency "," ostomy care efficiency "dimensions are higher than the control group,the difference is statistically significant(P <0.05).After repeated analysis of variance analysis,the difference between different time,groups and interaction effects was statistically significant(P <0.05).(4)At 12 weekends after discharge,the total score of the family function scale of the intervention group(2.29±0.34)was lower than that of the control group(2.41±0.24).The dimensions of "problem solving","communication","behavior control",and "total function" were lower than those of the control group,and the difference was statistically significant(P <0.05).The results of repeated measures analysis of variance showed that in the dimensions of "problem solving","family role","affective response","behavior control",and "total function",the differences were statistically significant at different times,between groups,and in interaction effects.(P <0.05).(5)At Twelve weekends after discharge,the total score of the quality of life scale(6.54 ± 1.04)of the ostomy patients in the intervention group was higher than that of the control group(6.34 ± 1.01);the overall quality of life scores of the two groups of patients after the intervention gradually changedwith different observation points improve.T test results showed that in the 12 weeks after the intervention,the intervention group was higher than the control group in the dimensions of "physical health","psychological health" and "spiritual health",the difference was statistically significant(P <0.05).Repeated analysis of variance analysis showed that the differences in time,group and interaction effects were statistically significant in the "mental health" and "mental health" dimensions(P <0.05).In the "physiological health" dimension,the difference was statistically significant at different times and between groups(P <0.05).Conclusions(1)Family-centered empowerment model Discharge plan can reduce the incidence of ostomy complications and intestinal complications in patients with prophylactic colostomy during hospitalization.(2)Family-centered empowerment model Discharge plan improves self-efficacy in patients with prophylactic colostomy.(3)Family-centered empowerment-based discharge plans can improve family function in patients with preventive colostomy.(4)A family-centered empowerment-based discharge plan can improve the quality of life for patients with prophylactic colostomy.
