| Objective:The aim of this study is to evaluate the impact of family empowerment program on the overall family environment of speech and hearing rehabilitation of children with cochlear implant,including parents’occupational stress,family management and family function,so as to provide reference for clinical nursing work.Methods:From October 2018 to December 2019,62 parents of children who started the cochlear implant operation in a third class a hospital in Changchun City were taken as the study objects.According to the time of the patients’cochlear implant in the outpatient department,they were divided into two groups:the final experimental group was 28 and the control group was 29.In the control group,routine outpatient education and follow-up were given,family rehabilitation instruction manual and rehabilitation diary were distributed.In the intervention group,on the basis of routine nursing,family empowerment program was systematically implemented for parents and family members of the children according to the process of family awareness,problem definition,discussion plan and implementation plan.Parenting Stress Index/Short Form,two subscales of Family Managemant Measure:disease management ability and disease management difficulty,and Feetham family function survey,were used to collect the data of the parents of the children when and 3 months after the start-up.SPSS 21.0 software was used for data analysis,measurement materials were described byx±s,and independent sample t-test was used for comparison;counting data were described by example,percentage,and chi square test was used for comparison;grade data were described by example,percentage,and nonparametric test was used for comparison,and P<0.05 was used to determine the difference as statistical significance.Results:(1)Before the intervention,there were no significant differences between the two groups in the scores of children’s and their parents’general information,parental stress score,disease management ability,disease management difficulty and family function score(P>0.05).(2)After 3 months of operation,the score of parental distress(Waldχ~2=22.379,P<0.001),parent-child interaction disorder(Waldχ~2=8.534,P=0.003),difficult children(Waldχ~2=22.938,P<0.001),and parental stress(Waldχ~2=55.811,P<0.001)in the intervention group were significantly lower.(3)After 3 months of operation,compared with the control group,the parents’disease management ability(Waldχ~2=5.256,P=0.022)of the intervention group was significantly improved,and the disease management difficulty(Waldχ~2=11.570,P=0.001)was significantly reduced.(4)After 3 months of operation,the family and subsystem relationship(Waldχ~2=10.045,P=0.002),family and individual relationship(Waldχ~2=10.154,P=0.001),family and social unit relationship(Waldχ~2=13.146,P<0.001),total family function table(Waldχ~2=23.026,P<0.001)of the intervention group were significantly improved.Conclusion:The family empowerment program of this study effectively reduces the parental pressure of parents of children with cochlear implant,improves the level of family management,improves the family function,and plays a positive role in the process of family speech and hearing rehabilitation of parents of children with cochlear implant.Medical staff and relevant organizations should ensure that the families of children with cochlear implant actively participate in the hearing and speech rehabilitation of children after operation.We can learn from this program,give more attention and support to parents of children with cochlear implant and the whole family,so as to promote the better rehabilitation and growth of children with cochlear implant. |
