Clinical Efficacy Evaluation Of Intra-articular Injection Of Platelet-rich Plasma In The Treatment Of Early And Middle-stage Knee Osteoarthritis

Objective : To compare the clinical efficacy of intra-articular injection of platelet-rich plasma and sodium hyaluronate in the treatment of early and middle-stage knee osteoarthritis,and to provide reference for the clinical treatment of early and middle-stage knee osteoarthritisMethods: From January 1st,2019 to June 30 th,2019,60 patients with knee osteoarthritis and K-L(Kellgren-Lawrence)Grade ?-? were selected from the outpatient department of Loudi Central Hospital.The patients were divided into two groups by random number table method,of which 30 patients who received Intra-articular injection of platelet-rich plasma were called platelet-rich plasma group and 30 patients who received Intra-articular injection of sodium hyaluronate were called sodium hyaluronate group.The basic data including age,sex,height,weight,knee joint side,previous treatment,visual analogue scale(VAS),Western and Mc Master Universities Osteoarthritis Index(WOMAC)were collected and analyzed.After all patients received complete intra-articular injection of knee joint,the pain VAS and WOMAC were obtained at the 1st,2nd,3rd,6th,12 th and 24 th weeks after treatment.The obtained data were statistically analyzed by SPSS19.0,and the two different treatments were evaluated according to the statistical results.The feelings and characteristics of patients after receiving treatment were recorded and summarizedResults : During the follow-up,one patient who received Intra-articular injection of platelet-rich plasma withdrew midway and underwent arthroscopic surgery.Two patients who received Intra-articular injection of sodium hyaluronate lost contact after receiving the first treatment.All the other patients who received the test obtained follow-up data through outpatient follow-up,telephone,We Chat,etc.After receiving all the treatments,VAS scores of the two groups were significantly different from WOMAC scores(P=0.000,P=0.000,P=0.000,P=0.000,P=0.000,P=0.000).It is believed that the two treatments are effective in relieving pain and improving daily behavior in the treatment of knee osteoarthritis in the early and middle stages.There was no significant difference in VAS scores between the two groups at the first and second weeks after treatment(P=0.598,P=0.072).At the 3rd,6th,12 th and 24 th weeks,the VAS scores of platelet-rich plasma group were lower than those of sodium hyaluronate group,and the difference was statistically significant(P=0.045,P=0.016,P=0.002,P=0.000).It is believed that platelet-rich plasma was better than sodium hyaluronate in relieving pain in the early and middle stages of knee osteoarthritis.There was no significant difference in WOMAC scores between the two groups at the first and second weeks after treatment(P=0.413,P=0.071).At the 3rd,6th,12 th and 24 th weeks,the WOMAC scores of platelet-rich plasma group were lower than that of sodium hyaluronate group,and the difference was statistically significant(P=0.024,P=0.017,P=0.007,P=0.001).It is considered that platelet-rich plasma was better than sodium hyaluronate in improving daily behavior in the treatment of early and middle knee osteoarthritis.Conclusion : Intra-articular injection of platelet-rich plasma and sodium hyaluronate can both relieve pain and improve daily behavior in the treatment of early and middle knee osteoarthritis.However,with the passage of time,the effect of platelet-rich plasma is more lasting,which can effectively relieve pain symptoms,and improve daily behaviors and quality of life of patients.It can be considered that Intra-articular injection of platelet-rich plasma can be used as a way to treat knee osteoarthritis in the early and middle stages..
