Effects Of FORT,AMH,AFC On Pregnancy Outcomes Of IVF/ICSI-ET In Infertility Patients

Objective : The study investigated three indicators of follicle output rate(FORT),anti-Müllerian hormone(AMH)and antra follicle count(AFC)for correlation and predictive value of pregnancy outcomes in infertile couples treated with in-vitro fertilization /intra-cytoplasmic sperm injection-embryo transfer(IVF/ICSI-ET)technology.Methods:The general data,embryo laboratory data and pregnancy outcome follow-up data of 390 controlled ovarian hyperstimulation cycles of infertile couples who received IVF / ICSI-ET treatment in our reproductive center from March 2016 to January 2019 were analyzed retrospectively.IBM SPSS Statistics 22 statistical software was used for data analysis.First,the clinical data included in the study are generally described.Then AMH and FORT are divided into three groups according to their values of < 25%,25%~75% and > 75%.AFC is divided into three groups according to their values of < 10,10~15 and > 15.The mean of clinical data among the groups were compared through analysis of variance.When p<0.05,we made a further comparison between the two groups.Then x 2 test was used to compare OHSS rate,embryo transfer rate,pregnancy rate,abortion rate and live rate.After that,Pearson correlation analysis was used to process the data between the number of occytes,pregnancy rate and the predictive indicators in this paper.The correlation analysis between the FORT,AMH,AFC and various clinical data was carried out.Finally,ROC curve analysis was used to evaluate the predictive value of FORT,AMH and AFC for OHSS rate,embryo transfer rate.Results:(1)Among the three indicators,FORT and AFC are positively correlated with AMH,respectively.FORT and AFC are negatively correlated.The correlation coefficient between AMH and AFC is 0.554 at the highest;(2)FORT,AMH and AFCwere positively correlated with OHSS.Among them,AMH had the highest prediction value,with AUC value of 0.758,the best prediction threshold point of 3.375 ng/ml,the sensitivity of 90.9%,and the specificity of 57.9%.FORT and AFC The significant prediction value of FORT and AFC is slightly lower;(3)FORT,AMH and AFC were positively correlated with the number of oocytes collected,and the difference was statistically significant(P< 0.05)between each index.The correlation coefficient between AMH and the number of oocytes collected was the highest,which was 0.472;(4)There was no significant difference in pregnancy rate,abortion rate and live birth rate between the groups of FORT,AMH and AFC(P > 0.05);(5)In the prediction of embryo transfer rate,the predictive value of AMH is higher than that of FORT and AFC.The AUC of AMH is 0.633,the best prediction threshold point of 3.25 ng / ml,the sensitivity is 56.7%,and the specificity is 66.7%.Conclusion:(1)FORT,AMH and AFC are correlated with each other,among which AMH is positively correlated with FORT and AFC,while FORT is negatively correlated with AFC.(2)FORT,AMH and AFC was more valuable in predicting ovarian response,that is,the prevalence of OHSS and the number of oocytes than in predicting pregnancy outcome.(3)The combination of FORT,AMH and AFC can guide the course of IVF/ICSI-ET treatment in predicting ovarian reactivity and embryo transfer rate,and they have some value in predicting pregnancy outcome,which still needs further study.
