BACKGROUNDDysphagia is a common complication of patients after acute ischemic stroke,and patients often suffer from various complications during the rehabilitation process of swallowing function.At present,because there is no standard rehabilitation treatment for dysphagia in clinic,a safe and effective rehabilitation treatment is urgently needed to improve the swallowing function of patients with dysphagia after acute ischemic stroke.Foreign studies have found that the natural capsaicinoids receptor of the transient receptor potential vanilloid 1(TRPV1)receptor agonist can promote the displacement of the hyoid bone,reduce the residue of swallowing,increase the speed of swallowing,and progressive dysphagia There is a good effect in the damaged elderly population.Volume viscosity swallow test(V-VST)is a method that can be used to screen for swallowing difficulties at the bedside.It has good reliability and validity,and can guarantee the effectiveness and naturalness of swallowing natural chili vegetarian groups in patients with dysphagia safety.No clinical application of natural capsaicin combined with V-VST technology has been found in patients with acute ischemic stroke.OBJECTIVE1.Through the surface electromyogram(sEMG),to determine the effect of the alimentary nectar bolus with natural capsaicinoids on the muscle promotion of the submental and infrahyoid swallowing muscles.2.To explore the safety and effectiveness of the alimentary bolus with natural capsaicinoids in patients with dysphagia after acute ischemic stroke,and provide an effective scientific basis for further clinical application.METHODSPart 1 sEMG evaluates the effect of the alimentary nectar bolus with natural capsaicinoids on patients with dysphagia after acute ischemic strokeIn this study,31 consecutive patients with dysphagia after acute ischemic stroke admitted to a department of neurology in a tertiary hospital in Guangzhou from February to April 2019 were included in the continuous sampling method.Eight healthy volunteers were recruited as the research object.Baseline control.Observe the peak and time-domain changes of sEMG in the submental and infrahyoid muscles muscles of the subjects during swallowing.Part 2 Safety and effectiveness of clinical application of the alimentary bolus with natural capsaicinoids in patients with dysphagia after acute ischemic strokeIn this study,120 consecutive patients with dysphagia after acute ischemic stroke admitted to the department of neurology in a top three hospital in Guangzhou from February to August 2019 were included in the continuous sampling method.Patients with dysphagia after stroke were randomly divided into a control group and a test group.The test group used a alimentary bolus with natural capsaicinoids for dietary intervention,and the control group used a thickener bolus for intervention.To observe the safety and effectiveness of the natural pepper paprika in patients with dysphagia after acute ischemic stroke.RESULTSPart 1 sEMG evaluates the effect of the alimentary nectar bolus with natural capsaicinoids on patients with dysphagia after acute ischemic strokeDuring the "dry swallowing" process,natural capsaicin can enhance the contractile strength of the submental and infrahyoid muscles(306.86±66.61 VS 420.15 ± 69.91 ?V,220.18 ± 28.12 ?V VS 295.23 ± 48.15 ?V,P<0.001)and shorten Muscle exercise time(1.513 ± 0.091 VS 1.37± 0.054 s,1.422± 0.0691 VS 1.334±0.099 s,P<0.001),the difference was statistically significant.During "10mL nectar bolus swallowing",natural capsaicin can enhance the contractile strength of the submental and infrahyoid muscles(323.02±53.78 VS447.89±92.80 ?V,197.74±51.12 VS 270.31±52.82 ?V,P<0.001)And shortening the muscle exercise time(1.370 ± 0.055 VS 1.276± 0.037 s,1.422 ± 0.069 VS 1.300± 0.040 s,P<0.001),the difference was statistically significant.Part 2 Safety and effectiveness of clinical application of the alimentary bolus with natural capsaicinoids in patients with dysphagia after acute ischemic stroke 2.1 Safety of natural capsaicin in patients with dysphagia after acute ischemic strokeBefore and after taking the alimentary bolus with natural capsaicinoids,patients'systolic blood pressure and heart rate were significantly changed.(139.04±12.55 VS 146.46± 11.57mmHg,P<0.001,75.21±8.07 VS 77.28 ± 7.36 times,P<0.001)The difference was statistically significant,but There was no significant difference in the effects of diastolic blood pressure and respiratory rate(78.25 ± 9.84 VS 78.00 ±9.32mmHg,P>0.05,19.32 ± 1.81 VS 19.55±1.66 times,P>0.05).The patient's tolerance to alimentary bolus with natural capsaicinoids was slightly worse than that of thickener bolus(7.75 ± 0.75 VS 4.43± 0.58,P<0.001),and the difference was statistically significant.2.2 Effectiveness of alimentary bolus with natural capsaicinoids in patients with dysphagia after acute ischemic strokeThe mRS at 1 month and 3 months in patients with dysphagia after acute ischemic stroke in the natural capsaicinoids group were smaller than those in the control group(2.92 ± 0.74 VS 3.27 ± 0.72,P=0.016,2.63 ± 0.63 VS 2.96±0.63,P=0.010),the difference is statistically significant.It had a small effect on the quality of life score of patients with dysphagia(177.58±21.30 VS 1 73.35 ± 23.77,P>0.05,193.75± 19.44 VS 187.35 ± 23.77,P>0.05),and the difference was not statistically significant.CONCLUSION1.Natural capsaicin can significantly improve the activities of the submental and infrahyoid muscles and shorten the swallowing response.2.Natural capsaicinoids can better promote the early recovery of dysphagia,and the prognosis is better,but the patient's tolerance to natural capsaicinoids is slightly worse.
