Objective:To explore the clinical effect of NPWT in the treatment of refractory pressure sore.Methods:The author reviewed and analyzed 46 patients with refractory pressure sore in our hospital from January of 2016 to September of 2019,According to the different treatment methods,they were divided into negative pressure treatment group and control group,The negative pressure treatment group applied negative pressure wound treatment technology(NPWT)after one-stage debridement,When the granulation tissue is fresh,the second stage is wound repair+vacuum sealing drainage;The control group was treated with conventional dressing change.After the granulation was fresh,the wound was repaired and vacuum sealing drainage was performed.In the negative pressure treatment group,there were 23 cases,including 13 males and 10 females,the minimum age was 41 years,the maximum age was 84 years,the average age was 72.70±2.15 years,There were 9 patients with 3-stage pressure sore and 14 patients with 4-stage pressure sore,the average time of wound formation was 74.83±4.65 days,and the average area of wound was 26.22±2.13cm2.In the control group,there were 23 cases,including 12 males and 11 females,with the minimum age of 56 years,the maximum age of 84 years,the average age of 74.91±1.37 years,There were 10 patients with 3-stage pressure sore and 13 patients with 4-stage pressure sore,the average time of wound formation was 73.43 ± 4.21 days,and the average area of wound was 23.74±1.43cm2.The wound healing,the negative rate of bacterial culture,the time needed for the operation of wound healing(skin grafting or flap sealing),the total time of wound healing,the C-reactive protein(C-reactive)before and after the first operation and before and after dressing change were observed Protein(CRP),procalcitonin(PCT),length of stay,total cost of stay and incidence of complications..Results:In the negative pressure treatment group,the negative rate of bacterial culture was higher than that in the control group,but the time of transition to the second stage of operation and the total time of wound healing were significantly shorter than that in the control group;in addition,the levels of C-reactive protein and procalcitonin before operation were also significantly higher than that after operation;the hospitalization time in the negative pressure group was shorter than that in the control group(P<0.05)Conclusion:Compared with the traditional dressing changing therapy,the negative pressure wound therapy(NPWT),It has the advantages of promoting wound healing,reducing postoperative complications and reducing hospital stay,which can reduce the workload of medical staff and patients' pain,and there is no significant difference in hospital cost between NPWT and traditional dressing change treatment.NPWT can be used as a routine treatment for refractory pressure sore,which is worthy of clinical application.
