Enteral Nutrition Intervention Of AIDS Patients With Impaired Immune Reconstitution Improves Their Nutritional Status

Objective(s):At present,HIV infection has become a long-term chronic infectious disease,following highly active antiretroviral therapy(HAART).As with most chronic diseases,both the disease itself and the drugs will have an impact on the patient's nutritional status over the course of continuous treatment.Malnutrition can be easily caused by a variety of reasons,and it will further aggravate the immune function damage,forming a vicious circle of immune deficiency-malnutrition in AIDS patients.This study aims to study the improvement of nutrition in AIDS patients with enteral nutrition intervention by using serological examination combined with human component analysis,thus providing new detection methods and effective treatment programs for the nutritional treatment process of AIDS patients.Methods:Sixty AIDS patients who met the inclusion requirements were included.The baseline values of nutritional indicators should be measured before enteral nutrition intervention,including blood routine,blood biochemistry,human body composition and other data.All patients were given a whole nutrition formula as a nutritional supplement,and the nutritional indicators of all patients were tested again at 1,2 and 3 months after the intervention.Results:1.The vast majority of AIDS with poor immune function reconstruction have malnutrition,of these,51 cases(85%)had mild or suspicious malnutrition and 7 cases(12%)had severe malnutrition.2.After intervention,the CD4+T cell and CD8+T count significantly increased(P<0.001),and the ratio of CD4/CD8 was unchanged.The levels of ALB was upregulated(P<0.05).The levels of TP,PA and GLB had no significant difference though under the intervention,However,the levels of TP and GLB decreased first and then increased during the intervention(P<0.01).The DBIL,BUN,Cre,Ca2+was degrade(P<0.05)and AST,LDL-C,Na+,serum phosphorus was upregulated(P<0.01).And there was no significant change on TBIL,IBIL,ALT,TC,HDL-C,K+,Cl-,Mg2+,serum iron.3.The levels of ECW,BMC,MINERAL,WEIGHT,VFA,AC,AMC was upregulated(P<0.05).There was no significant difference on the level of ICW,TBW,PROTEIN,SLM,FFM,FAT,SMM,BMR,BCM,BMI,PBF and WC.4.The nutritional risk score was negatively correlated with WEIGHT(r=-0.371,P=0.009),ICW(r=-0.356,P=0.012),ECW(r=-0.346,P=0.015),TBW(r=-0.352,P=0.013),PROTEIN(r=-0.353,P=0.013),MINERAL(r=-0.342,P=0.016),BMC(r=-0.349,P=0.014),SLM(r=-0.353,P=0.013),FFM(r=-0.352,P=0.013),SMM(r=-0.356,P=0.012),BMI(r=-0.381,P=0.007),BCM(r=-0.356,P=0.012),BMR(r=-0.352,P=0.013),AC(r=-0.428,P=0.002),AMC(r=-0.385,P=0.006),WC(r=-0.379,P=0.007).Conclusion(s):1.The vast majority of AIDS patients with poor immune reconstitution are malnourished,and some of them are severely malnourished.2.Human body composition analysis can reflect the nutritional status of AIDS patients,it can be used as a nutritional screening tool.The nutritional status of AIDS patients can be better evaluated by comprehensive use of nutrition screening scale,laboratory biochemical test and human body composition analysis.3.After the intervention with enteral nutrition agents,the immunologic function and the nutritional status of AIDS patients with poor immune reconstitution was improved.However,the well state of immune reconstruction has not been achieved.
