ObjectiveThe first part of this paper retrospectively analyzes the results of seven respiratory viruses and clinical data of hospitalized children under 5 years with acute respiratory infection(ARI)admitted to the Huaihe Hospital of Henan University from 2017 to 2019 to understand the virus The composition and epidemic characteristics of the pathogen spectrum provide the basis for clinical diagnosis and treatment.The second part is to observe the clinical efficacy of aerosolized rhIFN-?2b(recombinant human interferon alpha 2b)in the treatment of influenza-like illness,in order to optimize the treatment of children with Influenza-like illness.Methods1.Retrieve the medical records of children under 5 years old who were hospitalized in the pediatric department of Huaihe Hospital of Henan University from September 2017 to August 2019,and select cases in which seven respiratory viruses were detected using direct immunofluorescence with complete medical records.Retrospective analysis of epidemiological characteristics of Influenza A virus,Influenza B virus,respiratory syncytial virus,Adenovirus,Parainfluenza virus type1?3 in different age groups,genders and seasons.2.From March 2018 to February 2019,Pediatrics of Huaihe Hospital of Henan University recruited 180 cases of influenza-like illness between 1 and 14 years of age and were randomly divided into 3 groups:(1)observation group(60 cases),nebulized inhaled rhIFN-?2b;(2)positive control Group(60 cases),oral oseltamivir phosphate;(3)routine treatment group(60 cases),oral kanggan granules.The treatment was continued for 5 days and at home,and the child's guardian completed the treatment diary.According to the feedback of the children's treatment situation,172 cases of ILI who completed the treatment plan were screened out,and statistical analysis was performed to compare the main efficacy indicators,secondary efficacy indicators and the incidence of adverse reactions in each group.ResultsThe first part,the results of etiological analysis of respiratory viruses in children with acute respiratory infection in single center:1.A total of 1859 cases were included.The virus was detected in 401 children with a total positive rate of 21.57%,of which 387 cases were detected with a virus.The positive rates were RSV(9.95%),IBV(3.98)%),IAV(2.58%),PIV3(2.04%),ADV(1.61%),PIV1(0.48%),PIV2(0.16%).There were 14 cases of mixed infection,which accounted for 3.49% of positive cases.All seven respiratory viruses had mixed infections,and PIV infection was more common.2.The difference in detection rates of RSV,IV(including IBV and IAV)at different ages was statistically significant(P<0.05).RSV was highest in the 0-12 month age group,while IV was higher in the 37-60 month age group.There was no significant difference in positive infections of other viruses at different ages(P>0.05).3.The virus detection rate was highest in winter and lowest in summer.The difference in virus detection rate in different seasons was statistically significant(c2=46.160,P=0.000).RSV and IAV had high detection rates in winter,while ADV had high detection rates in summer,IBV had high detection rates in spring,and the differences were statistically significant(P<0.05).There were no statistically significant differences in respiratory viruses detected by different genders and diseases(P>0.05).The second part,the results of the efficacy of aerosolized recombinant human interferon alpha 2b in the treatment of influenza-like illness:1.There was no statistically significant difference in general information such as age,gender,and course of disease before admission between the three groups(P>0.05).2.The main efficacy index of the observation group,that is,the time for the body temperature to return to normal was shorter than that of the conventional treatment group,and the difference was statistically significant(P<0.05).The difference between the positive control group and the conventional treatment group was also statistically significant(P<0.05).At the same time,there was no significant difference between the observation group and the positive control group(P>0.05).3.In terms of relieving the symptoms of cough and sore throat,the observation group was better than the positive control group(P<0.05)and also better than the conventional treatment group(P<0.05).The effect of the positive control group on relieving cough and sore throat was better than that of the conventional treatment group(P<0.05).There was no significant difference in the healing time between the observation group and the positive control group,but the difference between the two groups was statistically significant compared with conventional treatment(P<0.05).4.There were no significant differences in the incidence of complications,antibiotic use,antipyretic drugs use,and incidence of adverse reactions among the three groups(P>0.05).ConclusionThe first part,the conclusions of etiological analysis of respiratory viruses in children with acute respiratory infection in single center:1.From 2017 to 2019,the most common viral pathogens of children under 5 years of age in ARI hospitalized at Huaihe Hospital of Henan University were RSV,followed by IV.The age of high incidence of RSV and IV infection was 0-12 months old and 37-60 months old.2.Winter is a season of high incidence of respiratory viruses.Both RSV and IAV viruses have the highest detection rate in winter.3.From 2017 to 2019,the highest detection rate of IBV is in spring.4.In the mixed virus infection,the parainfluenza virus is dominant.The second part,the conclusions of the efficacy of aerosolized recombinant human interferon alpha 2b in the treatment of influenza-like illness:Aerosolized recombinant human interferon alpha 2b can quickly relieve influenza-like symptoms,shorten healing time,and is safe and effective in the treatment of childhood influenza-like illness.Its clinical efficacy is similar to oseltamivir and is worthy of clinical application.
