Objective: To evaluate the efficacy and safety of recombinant human interferon a-1b in the treatment of bronchiolitis in infants with different routes of administration.Methods: 320 children with bronchiolitis were selected as the research object,the control group(Group A): atomization group 1(Group B): atomization group 2(Group C): intramuscular injection group(Group D)=1:1:1:1,each group of 80 cases.The control group(Group A): symptomatic treatment: according to the condition of giving antipyretics,oxygen inhalation,nebulized budesonide(Pulmicort)+ 2-/ beta receptor agonist or M receptor inhibitors.The atomization group: symptomatic treatment + rh IFN-alpha 1b inhalation,rh IFNalpha 1b dissolved in 3ml saline,adding compressed air atomizer for 10-15 minutes / time,rh IFN-alpha 1b usage: atomization group 1(Group B): 1ug/kg / time,2times / day,atomization group 2(Group C): 2ug/kg / time.2 times a day,each time the amount of not more than 40 ug.Intramuscular injection group (Group D): symptomatic supportive treatment + 1 ug /kg/ rh IFN-alpha 1b intramuscular injection,1 times / day.Comparison of clinical efficacy,clinical symptom disappearance time and efficacy score,hospital stay,lung function.Adverse reactions were recorded.Result: 1?Cough symptoms:(1)d1:B group,C group,D group and A group,children with cough score was not statistically significant(F=1.283,P=0.280);(2)in d2,d3,d4,d5,d6,d7,B group,C group,D group and A group,cough score lower,the difference was statistically significant(P<0.05);(3)in d2,d3,d4,d5,B group,D group,cough score higher than group C,the difference was statistically significant(P<0.05);(4)in d2,d3,d4,d5,d6,d7,B group and D group cough score difference was statistically significant(P>0.05).In the extent of cough remission,the atomization group and intramuscular injection group were significantly better than the control group,while the 2 groups were better.2?Pulmonary wheezing relief:(1)d1:B group,C group,D group and A group,there was no significant difference in children with wheeze score(F=0.959,P=0.413);(2)d2,d3,d4,d5,d6,d7,A group wheeze score compared with C group,D group,with statistical significance the difference(P<0.05);(3)d5,d6,d7,A group wheeze score higher than B group,the difference was statistically significant(P<0.05);(4)d2,d3,d4,C group wheeze score higher than D group and the differences were statistically significant(P<0.05).In the remission of wheezing,atomization group,intramuscular injection group was significantly better than the control group,2 groups of atomization,intramuscular injection group was better than the atomization of the 1 groups,and in the first 4 days of treatment,the atomization group was superior to the intramuscular injection group.3?Three concave syndrome remission:(1)there was no significant difference between d1:A group and B group,C group,D group of three children with concave illness score(F=1.920,P=0.126);(2)d2,d3,d4,d5,d6,d7,A group of three concave syndrome score compared with B group,C group,D group,has statistics the significance of difference(P<0.05);(3)d2,d3,d4,d5,B group,D group of three concave syndrome score was higher than that in group C,the difference was statistically significant(P<0.05);(4)d2,d3,d4,d5,d6,d7,no statistical significance B group and D group three concave syndrome score(P>0.05).The results showed that the atomization group and intramuscular injection group were better than those in the control group in the remission of hypoxia,but 5 days before the treatment,the atomization of the 2 groups was better than that of the atomization group and the intramuscular injection group(1).4?Defervescence time :(1)the difference of d1,d4,d5,d6,d7 groups were no significant difference in body temperature(P>0.05);(2)d2,d3:A group compared with B group,body temperature of C group,D group,the difference was statistically significant(F=3.782,P=0.011);(3)d1,d2,d3,d4,d5,d6,d7,no significant difference in body temperature of B group and C group,D group (P>0.05).The interferon group and intramuscular injection group were shorter than the control group.5?Improve lung function: TPEF/TE(1): d1: there was no significant difference in TPEF/TE ratio between the two groups(F=0.243,P=0.867);(2)the TPEF/TE ratio of d7:A group was lower than that of B group,C group and D group,the difference was statistically significant(F=225.825,P<0.001).Interferon atomization group and intramuscular injection group were better than control group in improving TPEF/TE.VPEF/VE:(1)d1: there was no significant difference in VPEF/VE ratio between the groups(F=2.040,P=0.108);(2)the VPEF/VE ratio of d7:A group was lower than that of B group,C group and D group,the difference was statistically significant(F=271.321,P<0.001).Interferon atomization group and intramuscular injection group were better than control group in improving VPEF/VE.6?Effective rate and hospitalization time:(1)the total effective rate: the effective rate of group A compared with B group,C group,D group,the difference was statistically significant(F=12.015,P=0.007);(2)the efficiency of RSV: the effective rate of group A compared with B group,C group,D group,the difference was statistically significant(F=12.825,P<0.001);(3)the total number of days in hospital: A group of hospital stay than that of B group,C group,D group,the difference was statistically significant(F=57.657,P<0.001),the D group compared with B group,C group was shorter.Interferon atomization group and intramuscular injection group were better than the control group in the total effective rate,the effective rate of RSV and the number of days in hospital,and the 2 groups were better than those in the atomization group and the intramuscular injection group in the days of hospitalization.7?The liver function of the four groups were in the normal range before and after treatment.Interferon treatment had no significant effect on liver function.Conclusion: Recombinant human interferon alpha-1b has obvious curative effect on infantile bronchiolitis,and the inhalation is better than intramuscular injection,it can significantly promote the recovery of children with bronchiolitis,the clinical symptoms and signs of life and shorten the course of disease,clinical curative effect is remarkable,safe,worthy of further promotion in clinical use.
