[Objective]:This study through to the yunnan province health adult male and adult patients with hemophilia living conditions and mental health status of system evaluation,the purpose is to understand without chronic diseases in yunnan province of adult males with adult hemophilia patient's quality of life and the difference between the mental health status,clear in yunnan province adult hemophilia patient's quality of life and the characteristics of the mental health status.It provides basic reference data for the quality of life and mental health(anxiety and depression)of adult hemophilia patients in yunnan province,and provides basis for subsequent psychological intervention of hemophilia patients to improve the overall quality of life.[Methods]:Retrospective study method was used,In September 2017 to December 2019 in the first affiliated hospital of kunming medical university and diagnosis of hemophilia in the first affiliated hospital of kunming medical university haematology department registration and management of adult patients with hemophilia 95 cases in random selection without chronic diseases,98 cases of normal adult men,through the questionnaire survey,telephone tracking,outpatient service,the length of follow-up data collection,collection of adult patients with hemophilia family economic conditions,marital status,education situation,treatment,bleeding,joint deformities is to collect,analyze into objects for quality of life,Using the health questionnaire(the MOS item short form-36 healthsurvey,SF-36),for its object to mental health situation analysis,the PHQ-9 depression scale,GAD-7 anxiety scale,information will be collected by the questionnaire of the patients using SPSS20.0 statistical software for data statistics analysis,preliminary discussion and yunnan normal adult male and adult hemophilia patient's quality of life and the relationship between the mental health situation.[Results]:1 General Information1.1 Age:There was no significant difference in age between the hemophilia group(30.±29.5 years old)and the non-hemophilia group(30.0±5.6 years old)(P>0.05).1.2 BMI:The BMI of the hemophilia group was 20.9±3.9(kg/m2)and that of the non-hemophilia group was 23.8±4.8(kg/m2),and the difference was statistically significant(P<0.001).1.3 Marital Status:In the hemophilia group,30 persons(31.60%)were married,62 persons(65.30%)were single and 3 persons(3.20%)had girlfriends;In the non-hemophilia group,36(36.70%)were married,38(38.80%)were single and 24(24.50%)had girlfriends.There was a statistically significant difference between the two(P<0.001).1.4 Education:In hemophilia group,there were 16(16.80%)in primary and secondary schools,27(28.40%)in junior middle schools,14(14.70%)in senior high schools and 26(27.40%)in universities.Others(junior college,vocational high school,graduate student)12(12.60%),non-hemophilia group primary and middle school 1(1.00%),middle school 5(5.10%),high school 8(8.20%),university 76(77.60%);Others(junior college,vocational high school,postgraduate)8(8.20%).There was a statistically significant difference between the two(P<0.001).1.5 Occupational status:In hemophilia group,there were 5 self-employed persons(5.30%),54 unemployed persons(56.80%),17 students(17.90%),12 employees(12.60%)and 7 others(7.40%);In the non-hemophilia group,there were 8 self-employed persons(8.20%),8 unemployed persons(8.20%),9 students(9.20%),36 employees(36.70%)and 37 others(37.80%).There was a statistically significant difference between the two(P<0.001).1.6 Family Income:Annual family income of hemophilia group(expressed as X):14 persons(14.70%)with x<10000 yuan;X?10000 YUAN,X ?50000 yuan 67(70.50%);X>50000 yuan,x<100000 yuan 9 persons(9.50%);5 persons(5.30%)with x? 100,000 yuan;Annual family income of the non-hemophilia group(expressed as X):X<10,000 yuan,12 persons(12.20%);X?10000 yuan,x?50000 yuan 33(33.70%);X>50000 yuan,x<100000 yuan 31 persons(31.60%);22 persons(37.80%)with X? 100,000 yuan.There was a statistically significant difference between the two(P<0.001).2 Comparison of quality of life and mental health between hemophilia group and non-hemophilia group2.1 Comparison of quality of life between hemophilia group and non-hemophilia group:Hemophilia in SF-36 set of physiological function(PF),physiological function(RP),body pain(BP),general health(GH),energy(VT)(SF),social function,emotional function(RE),mental health(MH)and health changes(HT)group scored lower on nine dimensions of hemophilia,statistically significant differences between(P<0.05 or P<0.001).2.2 Comparison of the scores of depression and anxiety between the hemophilia group and the non-hemophilia group:The scores of depression and anxiety in the hemophilia group were 8.66±7.459(points),6.95±6.432(points),and 5.93±5.302(points),and 4.76±4.949(points),respectively,with statistically significant differences(P<0.05).2.3 Comparison of the severity of depression and anxiety between the hemophilia group and the non-hemophilia group:Those with moderate or higher depression(40.00%)and those with moderate or higher anxiety(29.47%)in the hemophilia group were higher than those in the non-hemophilia group(17.34%and 15.30%),and there were statistically significant differences in the severity of depression between the two groups(P<0.05).There was no significant difference in anxiety severity between the two groups(P>0.05).3Comparison of quality of life and mental health of hemophilia patients with different degrees of severity3.1 Comparison of quality of life of hemophilia patients with different degrees of severity:The body function(PF)score of patients with severe hemophilia was 46.40±24.068(points)in SF-36 patients,lower than that of patients with moderate hemophilia 56.82±28.002(points)and mild hemophilia 70.0±26.220(points),with statistically significant differences(P<0.05).3.2 Comparison of scores of depression and anxiety in hemophilia patients with different severity degrees:There were no significant differences in depression scores(10.20±10.474),8.36±7.428,8.70±7.331),and anxiety scores(9.00±6.964),7.21±7.266,6.61±5.939)in patients with mild,moderate and severe hemophilia(P>0.05).3.3 Comparison of the severity of depression and anxiety among hemophilia patients with different degrees of severity:Patients with mild,moderate and severe hemophilia were more likely to suffer from moderate depression(40.00%,36.36%and 42.10%),and there was no statistically significant difference(P>0.05).Patients with mild,moderate,and severe hemophilia were likely to suffer from moderate or higher anxiety disorders(40.00%,27.27%,29.83%),with no statistically significant difference(P>0.05).4Comparison of quality of life and mental health of hemophilia patients with different treatment methods4.1 Comparison of quality of life of hemophilia patients with different treatment methods:The scores of sF-36 patients with physiological function of 19.01 ±33.130(points),physical pain of 40.42±23.730(points),and health change of 37.68±28.621(points)were lower than those with preventive treatment(40.63±44.729(points),54.00±26.165(points),and 52.08±30.321(points),with statistically significant differences(P<0.05).4.2 Comparison of scores of depression and anxiety among hemophilia patients with different treatment methods:The depression score and anxiety score of the on demand patients were higher than that of the prevention patients(8.13±7.374 and 6.75±7.674),and the difference was not statistically significant(P>0.05).5Comparison of quality of life and mental health of hemophilia patients5.1 Comparison of quality of life among different hemophilia patients:The scores of physiological function 27.11 ±38.494(points)and general health status 33.37±24.719(points)in patients with Hemophilia A were higher than those of patients with preventive treatment(6.82±22.613 points)and 16.82±21.009(points),with statistically significant differences(P<0.05).5.2 Comparison of scores of depression and anxiety among hemophiliacs of different types:The scores of depression and anxiety in hemophilia a(8.83±7.530)and anxiety(7.20±6.495)were higher than those in hemophilia b(7.00±7.266)and 5.00±6.181(5.00),and the differences were not statistically significant(P>0.05).6.Influencing factors of depression and anxiety:Sf-36 energy(B=-0.120,P<0.05)and health(B=-0.056,P<0.05)were negatively correlated with depression.Sf-36 energy(B=-0.132,P<0.05)was negatively correlated with anxiety.[Conclusion]:1.The quality of life of adult hemophiliacs in Yunnan province is worse than that of adult males without chronic diseases,and they are more likely to suffer from depression and anxiety,and the proportion of those with moderate depression or above is higher.2.The more serious hemophilia patients are,the worse their quality of life will be.But there was no difference in the likelihood of anxiety or depression among hemophiliacs of different severity.3.The quality of life of patients receiving on-demand treatment is worse than that of patients receiving preventive treatment.But there was no difference in the likelihood of anxiety or depression among hemophiliacs treated differently.4.There was a negative correlation between the quality of life of hemophiliacs and anxiety and depression.5.Increased screening for depression and anxiety in integrated care for adult hemophilia may improve overall health outcomes and treatment compliance.
