Objective: With the integration of medical and health resources and the continuous advancement of the construction of the medical union,it is important to improve the various systems of the medical union and maintain long-term motivation.As an important measure to promote hierarchical diagnosis and treatment,the main bodies of the medical consortium need to support each other and encourage each other.In this study,through the application of the relevant theories of medical association incentives,field investigations of the two types of incentive mechanisms of urban medical associations and county medical associations were conducted to understand specific external and internal incentive measures,identify existing problems and analyze the reasons,and propose improvements The external and internal rationality suggestions of the medical union incentive mechanism provide a reference for promoting the deep development of the medical union.Methods: At the beginning of this study,the basic conditions and operating characteristics of the medical federation were understood by consulting a large number of relevant documents and policies of the medical federation.Then through field survey to find out the specific incentives in the construction of medical consortium,to understand the implementation effect and the existing resistance.The on-site investigation is mainly divided into two forms.One is to hold forums and personal in-depth interviews to interview relevant personnel around the theme of incentive mechanisms.The second is to conduct questionnaire surveys on medical institutions,medical staff and patients to understand the operation status,incentive strength,and satisfaction of the medical consortium.The collation and analysis of the content anddata of the symposium and personal in-depth interviews will be conducted later.Finally,the scientific analysis and rationality of the core content of the incentive mechanism of this research are demonstrated through expert consultation and expert demonstration.Results: Summarize the effective external incentives of the medical consortium in the sample sites.The specific external manifestations at the government level increase the importance,establish the highest decision-making body(such as the council),and comprehensively manage the medical consortium affairs;increase financial investment for information construction and encourage medical Consortium agencies use information technology to solve problems such as lack of manpower and hardware;although some measures of the medical insurance department have not been fully implemented,they are still actively adjusting and promoting the development of medical consortia.The internal incentive mechanism is reflected in the resident presence of various types of medical personnel in leading hospitals,supporting lower-level medical institutions to improve service capabilities and increasing medical income;clarifying the source of salary subsidies for dispatched personnel to effectively handle salary management issues;rationally distributing revenue and expenditure balances,and using Certain economic incentives motivate both parties.Open green referral channels to promote the smooth implementation of two-way referrals.Summarizing the most effective incentive method for medical staff at the supplier is to increase wages and increase personal income.Increasing the opportunities for further studies and giving higher prospects can enhance the enthusiasm of medical staff for study and work.Increased patient satisfaction,improved doctor-patient relationship,and improved overall hospital image are a spiritual incentive for medical staff and can mobilize internal motivation.Patients on the demand side have a high degree of acceptance of the practice of sending experts from the leading hospital to the primary level for consultation,which can encourage them to stay in the area for treatment.Patients' overall satisfaction with the medical union is relatively high,but what needs to be improved is the completeness of medicines in the medical union,which affects the patient's way of seeking medical treatment and the quality of treatment.Conclusion: At present,there are some incentives in the medical federation,and there are still some problems to be solved.From the government-level medical insurance policy,salary system,performance evaluation and other aspects,it has not been developed with the medical federation.From the internal level of the medical federation,a reasonable benefit distribution mechanism has not yet been formed between members in most regions,and there is a significant gap between the number of referrals for patients and the like,which affects the volume polarity of the medical federation.Analysis from external reasons mainly includes that a reasonable coordination mechanism has not yet been formed between different vertical governments,and that the horizontal health department and the medical insurance have not reached a consensus on the development goals of the medical consortium.From the internal cause analysis,the motivations for the formation of members in the medical federation are inconsistent,and medical institutions at all levels have not really realized the integration of interests.Based on the theory of incentive compatibility,the medical consortium incentive mechanism should be perfected.Externally,the government's guidance and supervision should be coordinated,and government departments such as medical insurance,finance,and health should cooperate.Internal institutions need to maintain a balance of incentives between leading hospitals and lower-level medical institutions,and take multiple measures to motivate medical staff to motivate their work,increase incentives for patients,meet patient needs,and improve patient satisfaction.
